Children who discontinue the diet after achieving seizure freedom have about a 20% risk of seizures returning. The length of time until recurrence is highly variable, but averages two years. This risk of recurrence compares with 10% for resective surgery (where part of the brain is removed) and 30–50% for anticonvulsant therapy. Of those who have a recurrence, just over half can regain freedom from seizures either with anticonvulsants or by returning to the ketogenic diet. Recurrence is more likely if, despite seizure freedom, an electroencephalogram shows epileptiform spikes, which indicate epileptic activity in the brain but are below the level that will cause a seizure. I was wondering if insulin resistance would make my cholesterol go up on the ketogenic diet from a total of 220 before I went on it to 378 after being on it for six months. I have always been in a healthy weight range for my height, but I have always been extremely hungry most of the time. Try resistant starch. Resistant starch passes through the small intestine intact and therefore doesn’t count as a dietary carbohydrate. Instead, it travels to your large intestine, where it’s used to feed beneficial gut bacteria. Try adding a teaspoon of resistant starch, such as raw potato starch or green banana flour, to your keto smoothie each day to keep your gut happy.
How ketosis helps with epilepsy isn't known, but it does. Researchers at Johns Hopkins University studied 150 children with epilepsy in one important study. After a year on the ketogenic diet, half of children had 50% fewer seizures. One fourth of the children reduced their seizures by 90%. Great article! I have been diagnosed with familial hypercholesterolemia, however, I am not concerned at all. I continue to follow a restricted calorie, zero carb ketogenic diet, that I have been doing strictly for the last year and a half. All of my health markers are excellent, except for LDL particle number and total, and it is the large buoyant type. I am middle-aged (46). I did get both a CT scan and tri-vascular scan, and got perfect scores for both. That alleviated any concerns that I might have previously had. Since this diet has improved my gut issues, bodyfat, blood pressure, insulin, blood glucose, mood, energy, A1C, CRP, etc., - basically every single health parameter I have tested, I am sticking with it! I have studied fasting fairly extensively, done many 10-30 day fasts, and have said to my spouse if I ever got a cancer diagnosis I would immediately fast, but since learning more about ketogenic diets, which have most of the same benefits, I would go that route without question. I recently heard that chemotherapy is only successful in 40-50 % of the time. My impression is that the average person believes chemo and radiation are far more successful than they are in fact.
I would receive further support for my thinking during the summer of 1981, after completing my second year of medical school. That July, through one of my journalism contacts from my previous life, I had the opportunity to meet the controversial alternative cancer practitioner, the dentist Dr. William Donald Kelley. Over a 20 year period beginning in the early 1960s, Kelley had developed a very intensive nutritional approach to cancer that came under harsh public scrutiny and media attention when he agreed to treat Steve McQueen.

The traditional ketogenic diet, created for those with epilepsy consisted of getting about 75 percent of calories from sources of fat (such as oils or fattier cuts of meat), 5 percent from carbohydrates and 20 percent from protein. For most people a less strict version (what I call a “modified keto diet”) can still help promote weight loss in a safe, and often very fast, way.
A ketogenic diet differs dramatically from the carbohydrate-heavy Standard American Diet. When you eat a carbohydrate-rich meal, the ingested carbs are broken down into glucose. Glucose is then shuttled into cells by insulin, where it is used for energy production. The constant consumption of a high-carbohydrate diet causes the body to rely on glucose (sugar) for fuel, while rarely tapping into fat stores for energy. A ketogenic diet does just the opposite. It forces the body to turn to fats for fuel. A keto diet encourages the production of ketones, small water-soluble compounds, and the “burning” of fatty acids in adipose tissue (fat cells) for energy. Ketones are unique in that they are rapidly taken up by tissues and broken down to yield ATP, the primary energy currency of the human body. The process by which the body switches to using ketones for energy is referred to as “nutritional ketosis,” while the process of tapping into your body’s fat stores is termed “fat adaptation.”

Jimmy Moore: One thing we’ve not talked about, I alluded to a little bit earlier, is LDL particle size. When people hear LDL, they think “Oh that’s one number.” Uh-uh, it’s actually 2 major numbers, it’s a lot more than 2. It’s about 6 or 8 sub-fractions of LDL sizes. Primarily it’s pattern A and pattern B. Pattern A being the large fluffy kind. Pattern B being the small dense kind.

Type 2 diabetes. One study found that being on the keto diet for one year reversed diabetes for up to 60 percent of participants. With an average weight loss of 30 pounds, they dramatically reduced or eliminated their need for insulin and no longer needed oral hypoglycemic drugs. The keto diet is also easier to sustain than the calorie-restricted diet or the protein-sparing modified fast.
Low-carbohydrate high fat diets (LCHF diets) consistently improve all other markers of cardiovascular risk — lowering elevated blood glucose, insulin, triglyceride, ApoB and saturated fat (especially palmitoleic acid) concentrations, reducing small dense LDL particle numbers, glycated haemoglobin (HbA1c) levels, blood pressure and body weight while increasing low HDL-cholesterol concentrations and reversing non-alcoholic fatty liver disease (NAFLD).
I also want to be clear that I’m not making any specific recommendation here for treatment of cancer using ketogenic diet or anything else. As I’ve argued earlier in the podcast, I think cancer is a complex multifactorial disease and varies from individual. There aren’t any studies on control groups living healthy lifestyles, eating a low carb/whole food/meat diet, that shows an increase in cancer. There are 1000’s of examples of blood work showing improvements in LDL ratios (risk of heart disease) and low/steady blood sugar levels (diabetes prevention/treatment). That is actual science. as far as anecdotal evidence, there are many accounts of vegans/vegetarians that have became extremely ill over time due to lack of complete nutrition. It is also more difficult (takes more planning) to ensure you are getting sufficient amounts of calories since plant based is not calorie dense and it takes larger quantities. Fairly recently, the diet was introduced as a weight-loss diet by an Italian professor of surgery, Dr. Gianfranco Cappello of Sapienza University in Rome. In his 2012 study, about 19,000 dieters received a high-fat liquid diet via a feeding tube inserted down the nose. The study showed an average weight loss of more than 20 pounds in participants, most of whom kept it off for at least a year. The researchers reported a few minor side effects, like fatigue.
One of these patients, a woman from Appleton, Wisconsin, had been diagnosed in the summer of 1982 with stage IV pancreatic adenocarcinoma, the most aggressive form of this most aggressive disease. A liver biopsy during exploratory surgery confirmed the diagnosis of metastatic cancer, which the Mayo Clinic would later confirm. I believe (as well as the numerous experts I have interviewed) that over 90 percent of cancer cases are either preventable or treatable. The key here is to view cancer as a metabolic dysfunction, allowing you to gain control over this dreadful disease. Simply put, the right foods and strategies may help suppress cancer growth while simultaneously pushing it into remission.
A ketogenic diet typically leads to a broad range of improvements in blood lipids as well as other cardiovascular risk factors such as blood pressure and inflammation. However, the changes we see in total and LDL cholesterol levels are much less predictable than the changes in other blood lipids such as triglycerides and HDL cholesterol. For some people, calculated LDL cholesterol goes down, and for others it goes up, sometimes quite a bit. You can still eat plenty of healthy dietary fats and also include gut-healing foods like leafy and cruciferous greens, prebiotic-rich foods like garlic and dandelion greens, and probiotic rock stars including kimchi and unpasteurized sauerkraut. Even with a daily 50-gram carb allowance, you can find a lot of anti-inflammatory, gut-supporting foods.
Jimmy Moore: So close, so far that I think they really need to latch onto what the triglycerides and what the HDL really mean. When you’re eating too many carbs, it’s going to show up in the tricks. It totally will. When you’re not eating enough fat in your diet, especially saturated fat, this is such a key point. I know you hammer this in your videos, that’s why I love them. The saturated fat is so important to raising the HDL, and it’s that HDL that you want to have higher and yet it also shows up in your total cholesterol and then makes the doctors goes bat crap crazy when they see 220 on your total cholesterol, and the only thing that went up was HDL. Come on.

In part, keto diet weight loss is a real thing because high-fat, low-carb diets can both help diminish hunger and boost weight loss through their hormonal effects. As described above, when we eat very little foods that supply us with carbohydrates, we release less insulin. With lower insulin levels, the body doesn’t store extra energy in the form of fat for later use, and instead is able to reach into existing fat stores for energy.

A study with an intent-to-treat prospective design was published in 1998 by a team from the Johns Hopkins Hospital[20] and followed-up by a report published in 2001.[21] As with most studies of the ketogenic diet, no control group (patients who did not receive the treatment) was used. The study enrolled 150 children. After three months, 83% of them were still on the diet, 26% had experienced a good reduction in seizures, 31% had had an excellent reduction, and 3% were seizure-free.[Note 7] At 12 months, 55% were still on the diet, 23% had a good response, 20% had an excellent response, and 7% were seizure-free. Those who had discontinued the diet by this stage did so because it was ineffective, too restrictive, or due to illness, and most of those who remained were benefiting from it. The percentage of those still on the diet at two, three, and four years was 39%, 20%, and 12%, respectively. During this period, the most common reason for discontinuing the diet was because the children had become seizure-free or significantly better. At four years, 16% of the original 150 children had a good reduction in seizure frequency, 14% had an excellent reduction, and 13% were seizure-free, though these figures include many who were no longer on the diet. Those remaining on the diet after this duration were typically not seizure-free, but had had an excellent response.[21][22]
Most of the DNA errors that lead to cancer appear to modify cellular metabolism in cancer cells, by targeting a dozen or so signaling pathways. Metabolism is a term used to describe chemical reactions that help to keep our cells and, by extension, our body, alive. I have been on a low carb keto diet for more than a year. As T2DM my A1C dropped from 9% to 5.4% & I discontinued meds. All my lipids improved even with ample healthy saturated fat. More than a year now so I wonder why this would be a short term improvement when its obvious that I will not go back to a high A1C and taking 3 diabetes medications including sulphonylureas. It is clear from this article that you lack the necessary experience that would be gained from wholeheartedly trying the diet or monitoring patients doing it properly like me. Ignore it and take back your health by restricting carbs (<25 g a day) or as low as you reasonably can below 130g while being satisfied that you are getting adequate nutrition.
Often caused by lymph node removal or damage due to cancer treatment, lymphedema occurs because there’s a blockage in the lymphatic system and results in the swelling in leg or arm. A 2017 study involved patients who suffered from obesity and lymphedema and who embarked on a 18-week ketogenic diet. Weight and limb volume was significantly reduced. (5)

Everyone talks about upping their fats… I do not think that is the key to sweeping LDL out of the system. Upping cruciferous fiberous veggies… the fiber, vitamins and minerals contained in veggies bind with the LDL and move it on out. You would have to eat literally a truck load to make any serious dent in your daily carb allowance since most are very low net carb anyways.
Medications may be tapered and discontinued on an individual basis because one of the most common parental reasons for starting the diet is anticonvulsant reduction. However, not all children are able to come off medications. Some child neurologists tell families that the diet and medications are often a “partnership” in seizure control . Children should be seen in clinic every 3 months for the first year, with more frequent visits for infants and medically fragile patients.
Ketone bodies, acetoacetate, and β-hydroxybutyrate (βOHB), are byproducts of fatty acid oxidation in the mitochondrial matrix of the hepatocytes. There are many theories about the role of KB, but the existence of an anticonvulsant effect is controversial. Some authors have found no relationship between KB and synaptic transmission and seizure control.
I also might offer a thought as to why, from a more esoteric, more biochemical perspective, for most people diagnosed with cancer the ketogenic diet might not work. For the past 150 years, researchers have approached cancer as a disease in which perfectly happy, normal mature cells sitting in some tissue somewhere suddenly go awry, lose their normal regulatory restraint, develop a primitive, undifferentiated appearance or phenotype, begin proliferating without restraint, begin invading through tissues and organs, begin migrating, spreading, creating new blood vessels along the way to feed the rapacious appetite of cancer. But over the past 15 years, gradually, a new, more productive, and I believe more truthful hypothesis has emerged, spearheaded particularly by Dr. Max Wicha at the University of Michigan. Scientists such as Dr. Wicha have discovered that cancer may be a little more complicated than we have thought these long decades.
Fortunately, patients have an alternative—a ketogenic diet. Research indicates that a ketogenic diet improves multiple aspects of metabolic syndrome, inducing significant reductions in body fat percentage, BMI, hemoglobin A1c levels, blood lipids, and blood pressure. (10, 11, 12) The ketogenic diet produces these beneficial effects by reversing the pathological processes underlying metabolic syndrome, including insulin resistance and chronic inflammation.
The ketogenic diet for epilepsy is a very high-fat diet, with just enough protein for body maintenance and growth, and very low amounts of carbohydrate. In the ketogenic state, the body generates molecules called ketones (also called ketone bodies) when fats are metabolized. The goal of the KDE is for the brain to use ketones for energy rather than glucose as much as possible. Ketones are (largely) water-soluble, so they are easily transported to the brain. The brain cannot use fatty acids for energy, but it can use ketones for a large portion of its energy requirements.
Our normal healthy cells, be they situated in the brain or the skin of our feet, do prefer glucose as their primary energy source, obtained from the sugar circulating in the blood. That “blood sugar” comes from a variety of sources, including dietary carbohydrates occurring in fruits, starchy vegetables like potatoes, and grains. The complex carbohydrates in such foods are broken down into glucose during the digestive process, catalyzed by a variety of carb-specific enzymes like amylase.
In the study, Barbara A. Gower, Ph.D., and her colleagues analyzed data from 45 women diagnosed with ovarian and endometrial cancers. The women were randomly assigned to either a ketogenic or a standard, healthy diet group. The ketogenic group was asked to consume 70 percent of calories from fat, 25 percent from protein and 5 percent from carbohydrates. The comparison diet was one recommended by the American Cancer Society, high in whole grains and fruit and low in added sugar.

In this way, stem cells allow complex life to exist and continue, providing tissue replacements as needed, appropriate for the tissue in which they live. That is, liver stem cells will create new liver cells as needed, bone marrow stem cells will create new bone marrow clones as required, intestinal stem cells will form, as necessary, intestinal lining cells. In this way, the developmental capacity of stem cells seems to be governed by the local environment.
Right now as long as all of these other preponderant of everything is doing well … triglycerides low, HDL high, small LDL, lower … those are the key markers that you really want to be looking at. For some people cholesterol will go up. I would say, “Who cares?” What you want to pay attention to … blood sugar, you want to pay attention to triglycerides going under 70, you want to see if you can get HDL above 70, because all of these markers are really what matters more. Then again that HSCRP so you know where you’re standing in your inflammation. If your inflammation keeps going higher while you’re doing this, there’s something about what you’re doing that’s causing that inflammation.
Here are a few of the most common side effects that I come across when people first start keto. Frequently the issues relate to dehydration or lack of micronutrients (vitamins) in the body.
