Currently, after more than 25 years in practice, I am writing a two-volume set consisting of detailed case histories of our own patients, like the two mentioned above, to make the point that the therapy works in practice. For those diagnosed with poor-prognosis solid tumors, many now alive in excess of 10 years, I have prescribed a high carbohydrate diet, in total contradiction to what Dr. Seyfried proposes as the ideal anti-cancer approach.
The ketogenic diet achieved national media exposure in the US in October 1994, when NBC's Dateline television programme reported the case of Charlie Abrahams, son of Hollywood producer Jim Abrahams. The two-year-old suffered from epilepsy that had remained uncontrolled by mainstream and alternative therapies. Abrahams discovered a reference to the ketogenic diet in an epilepsy guide for parents and brought Charlie to John M. Freeman at Johns Hopkins Hospital, which had continued to offer the therapy. Under the diet, Charlie's epilepsy was rapidly controlled and his developmental progress resumed. This inspired Abrahams to create the Charlie Foundation to promote the diet and fund research.[10] A multicentre prospective study began in 1994, the results were presented to the American Epilepsy Society in 1996 and were published[17] in 1998. There followed an explosion of scientific interest in the diet. In 1997, Abrahams produced a TV movie, ...First Do No Harm, starring Meryl Streep, in which a young boy's intractable epilepsy is successfully treated by the ketogenic diet.[1]
Adhering to a keto diet can be challenging and may be particularly so for cancer patients, many of whom may be enduring side effects from treatment. Entering a state of ketosis requires following a strict diet-plan, comprised of high fat foods such as bacon, heavy cream, and butter, while simultaneously restricting other categories of food, such as starchy vegetables like sweet potatoes, whole grains, and certain fruits. This dramatic change in eating habits can lead to nausea and digestive upset in addition to unintentional weight loss and increased risk of malnutrition.
Along with slashing carbs, a ketogenic plan also calls for limiting your protein consumption. If you know your macronutrients, you recognize that cutting carbs and restricting protein means seriously upping your fat intake. And that’s exactly what a true ketogenic diet entails. “You’d want healthy fats to account for about 80% of your calories, and protein around 20%,” Westman says. (For comparison’s sake, the average American gets roughly 50% of her calories from carbs, 15% from protein, and 30% from fat, per the CDC.)

You can get cholesterol from eating animal foods like eggs, cheese, meat, and dairy but your body can also produce its own. An average 150 lb weighing male can synthesize 1000 mg of cholesterol a day. One single egg has 200 mg of cholesterol. The typical US dietary intake of cholesterol is about 307 mg[iv]. A systematic review of 26 short-term intervention trials (varying from 4-12 weeks) evaluated the appetites of overweight and obese individuals on either a very low calorie (~800 calories daily) or ketogenic diet (no calorie restriction but ≤50 gm carbohydrate daily) using a standardized and validated appetite scale. None of the studies compared the two diets with each other; rather, the participants’ appetites were compared at baseline before starting the diet and at the end. Despite losing a significant amount of weight on both diets, participants reported less hunger and a reduced desire to eat compared with baseline measures. The authors noted the lack of increased hunger despite extreme restrictions of both diets, which they theorized were due to changes in appetite hormones such as ghrelin and leptin, ketone bodies, and increased fat and protein intakes. The authors suggested further studies exploring a threshold of ketone levels needed to suppress appetite; in other words, can a higher amount of carbohydrate be eaten with a milder level of ketosis that might still produce a satiating effect? This could allow inclusion of healthful higher carbohydrate foods like whole grains, legumes, and fruit. [9]
Unfortunately, thanks to the widespread prevalence of the standard American diet (SAD) – characterized by high consumption of red meat, processed meat, pre-packaged foods, butter, fried foods, high-fat dairy products, refined grains, potatoes, corn, high-fructose corn syrup (HFCS), and high-sugar drinks – the incidence of obesity, insulin resistance, diabetes, cardiovascular disease, and many forms of cancer are skyrocketing in the U.S.

By late November, I had fallen from 197 lbs. to under 175 lbs., dropped from 28% to 18% body fat, and anecdotally felt much better in all aspects of my health. Drink lots of water. This is especially crucial on a low carb or keto diet. Why? When you eat carbohydrates, your body stores the extra as glycogen in the liver, where they are bound to water molecules. Eating low carb depletes this glycogen, which allows you to burn fat – but it also means you are storing less water, making it easier to get dehydrated. Instead of the traditional recommendation of 8 cups of water per day, aim for 16 cups when following a low carb lifestyle.

Meat – like grass-fed selections – and fresh veggies are more expensive than most processed or fast foods. What you spend on Keto-friendly foods will vary with your choices of protein source and quality. You can select less-expensive, leaner cuts of meat and fatten them up with some oil. With this case study, it is important to note that patients with GBM rarely experience a rapid tumor regression after surgical resection and conventional therapy. Thus, the researchers emphasized that the “…response of the GBM in this patient after standard treatment alone would be unlikely, further suggesting a role for targeting energy metabolism as part of the management strategy.”
If pricking your finger regularly isn't for you, you can also use ketone strips, which measure ketones in your urine. Some critics argue they aren’t as accurate as checking blood levels, but they can provide some indication of whether you’re in ketosis, they’re less expensive than glucose meters, and you don't have to prick your finger multiple times daily.
Chronic disease continues to ravage our world today despite tremendous advances in health care. Therapeutic approaches to treating this wide-range suffering cannot be met by technological growth in pharmacology, genetic therapy, or surgery. It should be obvious that the real solution for treating cancer and disease is not found in a man-made pill but rather is found in regulating the metabolic functions within our bodies.

Determining who will respond well to KD is a topic still ripe for research. Researchers have previously explored the use of electroencephalogram, but results from a recent study were not statistically signifigant.3 Other studies have explored a patient’s preference for certain foods, but neither have been very effective strategies to determine the success of KD.
The ketogenic diet is a natural, nontoxic metabolic therapy being studied and utilized for cancer prevention and treatment. It works because cancer cells are dependent upon a constant supply of blood sugar (glucose) to stay alive. Normal cells can make energy from both glucose and ketones (metabolic by-products of burning fat), but most cancer cells can only use glucose. Avoiding carbohydrates (starch and sugar) while enjoying delicious and healthy protein and fats will lower blood glucose and increase blood-ketone levels, resulting in a normal body state called nutritional ketosis. Research has shown that nutritional ketosis starves cancer cells while nourishing normal cells and strengthening total body health.
As of the moment, there is no industry standard as to how many calories should be consumed in a restricted ketogenic diet, but there are published studies that provide estimates. In one example, a 65-year-old woman who was suffering from glioblastoma multiforme (GBM), an aggressive type of brain cancer, was put into a restricted ketogenic diet that started with water fasting and then proceeded to consuming 600 calories a day only.
The most common side effect encountered is constipation. Many of the children who begin the diet are already prone to this problem because of limited mobility, hypotonia, or spasticity. Constipation can be treated with regular doses of polyethylene glycol, fiber, increased fluids, salt, mineral oil, intermittent pediatric-dose enemas, or magnesium hydroxide . Other more common side effects include hunger, acidosis (during illness), and hypoglycemia (just during the start of the diet). Many children as well can have gastroesophageal reflux, which can be managed with medications. The high fat content decreases gastric emptying, which promotes gastroesophageal reflux.

Aside from carb flu, be warned that staying in long-term, continuous ketosis may have drawbacks that may actually undermine your health and longevity. To stay on the safe side, I recommend undergoing a cyclic ketogenic diet. The "metabolic magic" that ketosis brings to the mitochondria actually occurs during the refeeding phase, not during the starvation phase.

Just from researching that I have been doing, this information seems to make a lot of sense and I may have found some information that is pertinent as to why many of you have experienced a rise in LDL levels. VLDL become LDLs when they drop the triglyceride. So, as we consume less carbs we start to lose triglycerides and the VLDLs start to become LDLs. I am no professional by any means, but as I read that article it made sense as well. It takes a while for some people’s livers to start to get the picture and send out more HDL to round up the LDL and there are things that we can do to aid that process such as adding olive oil and coconut oil to our diets as well as eating purple produce and exercising, but the LDL count will probably rise at first and slowly drop from thereafter. It will probably drop faster if we are taking steps to raise our HDL. The longer our body (liver) has been exposed to an unhealthy diet, the longer it will take to recover from it.

Having first read Dr. Price’s book during my journalism days, I knew that according to his exhaustive work, humans were a varied species, in the past living in and adapting to all ecological niches excepting the Antarctic, offering a variety of food sources. To me, his work offered a solution to the conflicting dietary advice even then being offered to the world. It didn’t make sense as Nathan Pritikin insisted or as Bob Atkins argued, that all humans should follow one specific type of diet: It just didn’t seem reasonable, to me at least.
To date, evidence from randomized controlled clinical trials is lacking, but needed, to answer the question of whether an adjuvant KD would benefit specific cancer patients. Human data pertaining to KDs and cancer are mostly based on single case reports and a smattering of preliminary clinical studies with small study cohorts, heterogenous study designs, poor compliance to the diet, noncomparable regimens, or without standardized dietary guidance. Even so, results of the first clinical studies support the hypothesis of an anti-tumor effect of KDs. For example, 10 of the 24 (42%) clinical studies included in a recent review [1] provide evidence for the anti-tumor effect of KDs, whereas seven (29%) showed no effect and only one study reported a pro-tumor effect of the KD. The currently available medical literature presents strong scientific evidence for the safe application of a KD only in patients with glioblastoma. However, a clear recommendation for adjuvant use of the KD in glioblastoma patients still requires results from ongoing randomized controlled clinical trials.

Infants and patients fed via a gastrostomy tube can also be given a ketogenic diet. Parents make up a prescribed powdered formula, such as KetoCal, into a liquid feed.[19] Gastrostomy feeding avoids any issues with palatability, and bottle-fed infants readily accept the ketogenic formula.[31] Some studies have found this liquid feed to be more efficacious and associated with lower total cholesterol than a solid ketogenic diet.[18] KetoCal is a nutritionally complete food containing milk protein and is supplemented with amino acids, fat, carbohydrate, vitamins, minerals and trace elements. It is used to administer the 4:1 ratio classic ketogenic diet in children over one year. The formula is available in both 3:1 and 4:1 ratios, either unflavoured or in an artificially sweetened vanilla flavour and is suitable for tube or oral feeding.[51] Other formula products include KetoVolve[52] and Ketonia.[53] Alternatively, a liquid ketogenic diet may be produced by combining Ross Carbohydrate Free soy formula with Microlipid and Polycose.[53]


The ketogenic diet appears to enhance mitochondrial function via a number of potential pathways. Given the important role of mitochondrial dysfunction in many neurodegenerative diseases, it is important to outline potential mechanisms of apparent disease-modifying effects of the ketogenic diet. It is unclear whether there is something specific or direct about the ketogenic diet (ie, provision of ketone bodies or fatty acids) or, perhaps more importantly, the metabolic changes it induces.

Unfortunately that’s wrong. Unfortunately total cholesterol doesn’t tell the whole story. It includes one number that you want to have higher. That is you HDL good cholesterol. When you start eating low carb, high fat, keto, one of the tell tale signs that you’ve done it very well is your HDL, especially you ladies, you’re lucky Leanne … you ladies can make your HDL just go really high. I have to work hard as a guy to get mine in the 70-80 range, which is pretty good for a guy. Most people walking around their HDL is sub 40, and most of them probably sub 20 if they’re not eating enough fat. You have to eat saturated fat in order to raise that good HDL cholesterol. Okay?
A ketogenic diet is a very high-fat low-carbohydrate diet that is designed to induce fat metabolism. When the body is depleted of glucose stores, it shifts to metabolizing fat and fatty acids, which produces compounds called ketones. Ketones cross through the blood-brain barrier and enter the brain, where they’re used as an alternative energy source.
Dr. Campos, it is unfortunate that you retain the medical community’s negative stance on the ketogenic diet, probably picked up in medical school when you studied ketoacidosis, in the midst of an obesity and type II diabetes epidemic that is growing every year, especially among populations who will never see the Harvard Health Letter. The medical community has failed in reversing this trend, especially among children, and the public is picking up the tab, in the form of higher health insurance premiums to treat chronic metabolic diseases which doctors cannot cure. The ketogenic diet does not bid its adherents to eat unhealthy processed meats, and the green leafy vegetables that it emphasizes are important in a number of nutritional deficiencies. People lose weight on the ketogenic diet, they lose their craving for sugar, they feel more satiety, they may become less depressed, their insulin receptors sensitivity is improved, and these are all the good outcomes you fail to mention. There is a growing body of research which demonstrates the neuroprotective effects of the ketogenic diet to slow cancer progression, as well as diseases like Parkinson’s and Alzheimer’s, for which there are no effective medical treatments. Please respect your patients by providing them with evidence-based medical outcomes, not opinions.
Insulin is a hormone that lets your body use or store sugar as fuel. Ketogenic diets make you burn through this fuel quickly, so you don’t need to store it. This means your body needs -- and makes -- less insulin. Those lower levels may help protect you against some kinds of cancer or even slow the growth of cancer cells. More research is needed on this, though.
The research on how extended intermittent fasts affect cancer patients backs up our biochemical understanding as well. In initial case studies, cancer patients who were undergoing chemotherapy voluntarily fasted for anywhere between 48 to 140 hours (much longer than the intermittent fasts that keto dieters typically do). Each person reported fewer side effects and an improved quality of life regardless of how long they fasted.
The present study shows the beneficial effects of a long-term ketogenic diet. It significantly reduced the body weight and body mass index of the patients. Furthermore, it decreased the level of triglycerides, LDL cholesterol and blood glucose, and increased the level of HDL cholesterol. Administering a ketogenic diet for a relatively longer period of time did not produce any significant side effects in the patients. Therefore, the present study confirms that it is safe to use a ketogenic diet for a longer period of time than previously demonstrated.
The idea behind ketogenic diets is very simple. If glucose is the primary fuel for cancer, then lower carbohydrate intake and replace carbohydrates with other sources of fuel, such as fats, in order to push the body’s metabolism into ketosis. It actually turns out that ketogenic diets are probably useful in the treatment of intractable epileptic seizures in children. Unfortunately, their mechanism of action in preventing seizures is unclear, although four potential mechanisms, including carbohydrate reduction, activation of ATP-sensitive potassium channels by mitochondrial metabolism, inhibition of the mammalian target of rapamycin (mTOR) pathway, or inhibition of glutamatergic synaptic transmission (glutamate as a neurotransmitter), have been proposed. Interestingly, the mTOR pathway is an important signaling pathway in many cancers that couples energy and nutrient abundance to the execution of cell growth and division, owing to the ability of TOR protein kinase to simultaneously sense energy, nutrients and stress and growth factors. It’s a commonly overactive signaling pathway in cancer.
The Johns Hopkins Hospital protocol for initiating the ketogenic diet has been widely adopted.[43] It involves a consultation with the patient and their caregivers and, later, a short hospital admission.[19] Because of the risk of complications during ketogenic diet initiation, most centres begin the diet under close medical supervision in the hospital.[9]
Leanne: Yeah totally, chill out. Do a little meditation if you need to get into the zone. Exactly. You mentioned vegetable oils causing inflammation, are there specific fats that will help us, like we’ve talked a little bit about saturated fats, we all know trans fats don’t even go there … the mono poly saturated, unsaturated, is there a certain ratio that we should be aiming for. I know you’re the guy that will just slice off butter and eat it … I am too, a little bit of Himalayan rock salt on there never hurt anyone. Are there specific facts that we should be focusing on?
Jimmy Moore: Exactly. You’re already have a propensity for insulin resistance. It should not surprise you that as you get into this that you’re going to run into some of the manifestation of that, which is this thing called Don Phenomena, where in the morning you have a higher level of blood sugar, that’s totally normal. What tends to happen though, and certainly the people that have asked that question of you, I hope they’re testing later in the day as well. What you’ll see is later in the day, totally normal.
Jimmy Moore: Yeah. The next book that I’m going to write with my co-author, Dr. Eric Westman, not anytime soon because I’m tired of writing books right now … when we get back to it, it’s going to be on this blood sugar topic because it is one that is so confusing to people and I think focusing so intently on blood sugar is the wrong question to ask. I think we should be looking at blood insulin. I think that will be the tell tale sign of what’s really going on in your body. Blood sugar is going to do what it’s going to do. The body is trying, especially if you’re insulin resistant which a lot of people coming into a low carb, high fat diet are, hello …

The authors defined a VLCKD as a diet lower in 50g of carbohydrates – lower than the daily recommended grams of carb consumption clinicians recommend to diabetics. They included 13 randomized controlled studies with a total of 1,415 subjects. All studies took place for at least a full year and all subjects included were over 18 years old and had a BMI of at least 27.5 kg/m2. In each of these studies, VLCKD diets were compared to low-fat diets.
