The ketogenic diet field is moving in the direction of adding vitamins and supplements empirically to the regimen of all children on the ketogenic diet to prevent adverse effects before they occur. Although not mentioned in the 2009 consensus statement, many now advocate for universal treatment with oral citrates, selenium, extra vitamin D, laxatives, and even antacids . Several nutrition companies have created products which are designed for ketogenic diet patients.
Now, there’s even evidence that a low-carb, high-fat regimen (as the keto diet is) helps you live longer, compared to a low-fat diet. In a study by the medical journal The Lancet that studied more than 135,000 adults from 18 countries, high carbohydrate intake was associated with higher risk of total mortality, whereas total fat and individual types of fat were related to lower total mortality. Total fat and types of fat were not associated with cardiovascular disease, myocardial infarction or cardiovascular disease mortality.
A: It's generally recommended that only 5 percent of your daily diet is allocated to carbohydrates because if you consume more than that, your body gets thrown off ketosis. However, this is only for SKD, or the standard ketogenic diet. If you're an athlete or a bodybuilder, you can consume more carbs without affecting ketosis by following a targeted ketogenic diet (TKD) or a cyclic ketogenic diet (CKD).
Epilepsy is a disabling and common neurological disease, which can be controlled successfully in most patients with one or more antiepileptic drugs. Approximately 30% of patients with epilepsy have refractory epilepsy, that is, have a failure of adequate trials of two tolerated, appropriately chosen and used antiepileptic drug schedules to achieve sustained relief of seizures (Picot et al., 2008; Kwan et al., 2009). Some of these patients are not surgery candidates, so it is necessary to search for alternative treatments for epilepsy such as palliative surgery, neuromodulation, and a ketogenic diet (KD).

Now intrigued, I asked why he would want to change jobs, since our practice was by design slower paced, whereas Bob ran a very busy clinic and active IV unit which would seem perfectly suited for this nurse’s expertise. He then explained, with obvious disappointment, that none of the hundreds of cancer patients they had treated or had been treating had responded to any significant degree, with the exception of those he had referred to me.


There are thousands of people out there who have healed cancer naturally. I meet natural survivors constantly and even share their stories on this site. Most natural cancer healing protocols involve a radical change of diet and lifestyle that includes “overdosing on nutrition” with juicing, lots of raw plant food, little to no animal food, supplements, and herbal cleanses along with detox protocols. Those are all time-tested methods validated by a large body of long-term survivors.
Tumors did not progress at all at all in the five patients that successfully completed the ketogenic trial. This is a positive outcome given the advanced stage of their cancer. Additionally, some of these patients experienced favorable changes in glucose, HDL:LDL ratio, triglycerides, and healthy levels of weight-loss. These findings further support the healthy impact a ketogenic diet may have on cancer.
Another memorable patient written up for the book had been diagnosed with what was thought to be localized endometrial cancer in 1969. After a course of radiation to shrink her large tumor, she underwent hysterectomy, and was told they “got it all.” Over the next few years, however, her health began to deteriorate: she experienced persistent fatigue, malaise, pelvic pain, and weight loss.

Feldman’s theory about why this happens is based on research he’s conducted on himself and hundreds of other low-carbers over the past two years. He states that the higher energy demands, lower body fat stores, and lower glycogen stores in these LMHRs trigger the liver to increase production of lipoprotein particles so that triglycerides (fat) can be transported to cells for use as fuel. Since cholesterol travels along with the triglycerides, blood cholesterol levels rise as the liver pumps out more lipoproteins to keep up with the body’s energy demands.
The authors defined a VLCKD as a diet lower in 50g of carbohydrates – lower than the daily recommended grams of carb consumption clinicians recommend to diabetics. They included 13 randomized controlled studies with a total of 1,415 subjects. All studies took place for at least a full year and all subjects included were over 18 years old and had a BMI of at least 27.5 kg/m2. In each of these studies, VLCKD diets were compared to low-fat diets.

Although the exact role of the keto diet in mental and brain disorders is unclear, there has been proof of its efficacy in patients with schizophrenia. And, to boot, it works to reverse many conditions that develop as a side effect of conventional medications for brain disorders, like weight gain, type 2 diabetes and cardiovascular risks. More research is needed to understand the role of the ketogenic diet in treating or improving schizophrenia, as the current available studies are either animal studies or case studies, but the benefits of a low carbohydrate, high-fat diet in neurology is promising.
The Johns Hopkins Hospital protocol for initiating the ketogenic diet has been widely adopted.[43] It involves a consultation with the patient and their caregivers and, later, a short hospital admission.[19] Because of the risk of complications during ketogenic diet initiation, most centres begin the diet under close medical supervision in the hospital.[9]
If you’ve decided to move forward in trying the keto diet, you will want to stick to the parameters of the eating plan. Roughly 60 to 80 percent of your calories will come from fats. That means you’ll eat meats, fats, and oils, and a very limited amount of nonstarchy vegetables, she says. (This is different from a traditional low-carb diet, as even fewer carbs are allowed on the keto diet.)
The popular low-carb diets (such as Atkins or Paleo) modify a true keto diet. But they come with the same risks if you overdo it on fats and proteins and lay off the carbs. So why do people follow the diets? "They're everywhere, and people hear anecdotally that they work," McManus says. Theories about short-term low-carb diet success include lower appetite because fat burns slower than carbs. "But again, we don't know about the long term," she says. "And eating a restrictive diet, no matter what the plan, is difficult to sustain. Once you resume a normal diet, the weight will likely return."
Scientists published the first pilot study on the relationship between the ketogenic diet and cancer in 2011. For their study, they recruited 16 patients (12 women and 4 men) with various cancers in advanced stages. They had an average age of 50.4 years (30-65 years). All of them had metastatic tumors, which means that they grew and spread throughout the body.
In the first week, many people report headaches, mental fogginess, dizziness, and aggravation. Most of the time, this is the result of your electrolytes being flushed out, as ketosis has a diuretic effect. Make sure you drink plenty of water and keep your sodium intake up.6One of the fathers of keto, Dr. Phinney, shows that electrolyte levels (especially sodium) can become unbalanced with low carb intake.

Anticonvulsants suppress epileptic seizures, but they neither cure nor prevent the development of seizure susceptibility. The development of epilepsy (epileptogenesis) is a process that is poorly understood. A few anticonvulsants (valproate, levetiracetam and benzodiazepines) have shown antiepileptogenic properties in animal models of epileptogenesis. However, no anticonvulsant has ever achieved this in a clinical trial in humans. The ketogenic diet has been found to have antiepileptogenic properties in rats.[56]
Some people on a keto or low carb diet choose to count total carbs instead of net carbs. This makes it more difficult to fit in more leafy greens and low carb vegetables (which are filled with fiber), so you should only try that if you don’t get results with a net carb method. And, start with reducing sugar alcohols and low carb treats before deciding to do a “total carbs” method.
According to a 2015 review of the literature on the ketogenic diet for human glioma patients (32 case studies), “Prolonged remissions ranging from more than 5 years to 4 months were reported in the case reports. Only one of these patients was treated using KD as monotherapy. The best responses reported in the more recent patient series were stable disease for approximately 6 weeks.”

A survey in 2005 of 88 paediatric neurologists in the US found that 36% regularly prescribed the diet after three or more drugs had failed, 24% occasionally prescribed the diet as a last resort, 24% had only prescribed the diet in a few rare cases, and 16% had never prescribed the diet. Several possible explanations exist for this gap between evidence and clinical practice.[34] One major factor may be the lack of adequately trained dietitians, who are needed to administer a ketogenic diet programme.[31]
If you have a history of hypothyroid issues, you may also struggle with unhealthy cholesterol levels as well — and the keto diet can make them even worse. However, for those of you who are being treated for your hypothyroid condition or who have an autoimmune thyroid condition, you may be able to follow the keto diet without any problems. In fact, many keto dieters with autoimmune thyroid conditions have found that the keto way of eating improved their quality of life more than any other diet.

The ketogenic diet now has nearly 100 years of history supporting its use. Most major medical textbooks and reviews no longer question its efficacy. About 50% of the children appear to have had an excellent response to the diet as defined by marked reduction (at least a 50% reduction) in seizure activity. About 15% of children become seizure-free after initiation of the ketogenic diet, usually within 3-6 months, and typically stop the diet after 2 years. Of those children who become seizure-free, a minority (15-20%) have recurrence after the diet is stopped. Risk factors that increase the likelihood of recurrence of seizures are:
Anecdotally, I’ve spoken with some cancer researchers who claim to be virtually curing cancer in animals using a combination of ketogenic diet and PI3K or mTOR, like rapamycin, but these data aren’t published. Again, we need to be cautious about accepting these claims until they’ve gone through the legitimate scientific channels and people have had a chance to review this research.
"Findings from our study reveal that treating mice chronically with specific bacteria that were enriched by the ketogenic diet protected them from seizures," study senior author Elaine Hsiao, an assistant professor of integrative biology and physiology at the University of California, Los Angeles (UCLA), told Live Science. ("Keto diet" is short for "ketogenic diet.") However, Hsiao stressed that more studies are needed before researchers know if the findings also apply to people.
‘”Absolutely not… A number of situations where the ketogenic diet may not be the preferred therapy for most cancers, I would say, leukemia, lymphomas, Hodgkin’s lymphoma, thyroid cancer, testicular cancer, if caught early prostate cancer, melanoma, breast cancer. All these cancers can be effectively treated with chemotherapy or radiation in some cases, and also brain tumors if it’s grade 1 or 2 tumor that’s not very metastsatic and is more localized then surgery, radiation, and chemo can be very effective.”
A short-lived increase in seizure frequency may occur during illness or if ketone levels fluctuate. The diet may be modified if seizure frequency remains high, or the child is losing weight.[19] Loss of seizure-control may come from unexpected sources. Even "sugar-free" food can contain carbohydrates such as maltodextrin, sorbitol, starch, and fructose. The sorbitol content of suntan lotion and other skincare products may be high enough for some to be absorbed through the skin and thus negate ketosis.[31]

In general, cholesterol is traditionally misunderstood. I am going to show why high cholesterol is not inherently bad, and how to lookout for some real warning signs that you are in an inflammatory state. Knowing how cholesterol works in the body and how to interpret your cholesterol numbers will empower you to move forward on your ketogenic journey with confidence.
Taken together, results from preclinical studies, albeit sometimes contradictory, tend to support an anti-tumor effect rather than a pro-tumor effect of the KD for most solid cancers. However, even though pro-tumor effects are rare, they cannot be ruled out per se. Most importantly, available preclinical evidence implies that the feasibility of a KD as an adjuvant cancer therapy strongly depends on the type of tumor and its genetic alterations.
Eliminating several food groups and the potential for unpleasant symptoms may make compliance difficult. An emphasis on foods high in saturated fat also counters recommendations from the Dietary Guidelines for Americans and the American Heart Association and may have adverse effects on blood LDL cholesterol. However, it is possible to modify the diet to emphasize foods low in saturated fat such as olive oil, avocado, nuts, seeds, and fatty fish.
Data on the efficacy of KD and the modified Atkin’s diet (MAD) in children and adults are similar.1 “Approximately one-third of adults with epilepsy that does not respond to medications may have a 50% seizure frequency reduction. Five to 10% of these adults may have a dramatic response, with greater than or equal to 90% seizure frequency reduction. This is similar to the response seen in children,” said Klein.
With renewed use of the ketogenic diet has come heightened interest in its potential use for other conditions (Table 1). Over the past few years, there has been an explosion in speculation about the diet’s potential applications in a variety of metabolic, oncologic, neurodegenerative, and psychiatric disorders. This review examines data supporting the potential use of the ketogenic diet in each disorder and considers potential mechanisms of action in each disorder, using these data to shed light on the diet’s disease-modifying effects. Both the human and animal studies discussed used standard ketogenic diets unless otherwise specified.
Being in ketosis can help you feel more alert, avoid sugar crashes that steal your energy, and possibly let you reach your goal weight. I say possibly because it doesn’t work for everyone. If you’re eating too many inflammatory calories you won’t lose weight, and furthermore, some people get bored with the lack of variety on keto diets. That’s where eating more complex carbs can rescue the plan. Try cyclical ketosis.
After scouring the literature, he became quite attracted to the “good science” behind the ketogenic hypothesis, so under Dr. Seyfried’s direct supervision, he began the diet. Though the patient seems quite enthusiastic about his response, he admits in his note that with the diet there has been “no progression,” presumably in terms of x-ray studies, and some improvement in the blood studies. He still considers his disease as “incurable.”
Tony, I'm not sure how you were only eating 20 grams of fat on a keto diet; in fact, if this is true, that may be the reason. A keto diet should provide a minimum of 70 grams of fat daily, but generally 100+ grams for most people, especially men. Be sure to include nuts, olive oil, avocados, fatty fish, and other healthy sources of fat in your diet on a daily basis in order to prevent problems. - Franziska

We decided to join a ketogenic diet study. Not something you would have expected for cancer treatment. This wasn’t a random decision because there are many studies looking at how diets might improve cancer outcomes. I joined Alina as a coach and chef. You probably have heard about the “ketogenic” diet. It consists of lots of fat, some protein, and minimal carbs. Using this diet, our body switches from glucose as a fuel source to ketones. Carbs are strictly for those must-have nutrients.
You want to keep your cheats to none. Be prepared, make sure you’re eating what you need to be satiated (“full”), and make sure you’re satisfied with what you’re eating. If you have to force yourself to eat something, it will never work out in the end. This is just a guideline on how you can eat on a ketogenic diet, so you’re very welcome to change up what kind of foods you eat!
A recent study found that ketone supplementation extended survival in mice with metastatic cancer. But while it’s true that most cancers have a highly anaerobic metabolism, this in not universal. If proven to be effective, it’s likely that ketone supplementation would be an additional treatment rather than a stand alone treatment for cancer, because of its robust nature.
A ketogenic diet is a very high-fat low-carbohydrate diet that is designed to induce fat metabolism. When the body is depleted of glucose stores, it shifts to metabolizing fat and fatty acids, which produces compounds called ketones. Ketones cross through the blood-brain barrier and enter the brain, where they’re used as an alternative energy source.

By that point, I knew Dr. Good quite well: during my second year of medical school, Dr. Good, at the time a professor at Cornell and Director of the Sloan-Kettering Institute, had begun guiding my fledgling research career. In 1982, during my third year of medical school, to my dismay the powers that be at Sloan pushed him out rather unceremoniously.
"It's a cocktail of drugs and procedures and foods and they all work synergistically to gradually eliminate the tumor while maintaining the health and vitality of our normal organs. The whole goal of this metabolic therapy, which involves the ketogenic diet, is to gradually degrade and eliminate tumor cells without toxicity so the patient emerges from the therapy healthier than when they started."
It was late 1985 when the media broke the story about the next cancer miracle. I was sitting in my apartment overlooking beautiful Tampa Bay, when I read the initial front-page newspaper reports. Dr. Steven Rosenberg, already well-known as Ronald Reagan’s surgeon (the President had a malignant polyp), and a highly regarded basic science researcher running a section at the National Cancer Institute in Bethesda, Maryland, had just revealed to the world – at a press conference, as I remember – his preliminary pilot study results with a new immune modulator, interleukin-2, that would provoke an extraordinary media frenzy.

And don’t associate yourself with Ralph Moss, the number one promoter of laetrile quackery and make easily refuted claims such as the claim that “chemo and radiation do not cure cancer or extend life, although cancer physicians often make this claim” and that radiation “often does more harm than good to the patient.” Given that all Dr. Seyfried has is a couple of case studies as clinical support for his treatment (see below) and I can produce reams of studies over nearly 50 years demonstrating that chemotherapy can cure specific cancers and prolong life when used appropriately, the “2% gambit” notwithstanding, it’s not a winning proposition, and it sure doesn’t help your credibility to use the language of cancer quacks to promote your idea.

• Standard ketogenic diet (SKD) — SKD is the type I typically recommend for most people, because it is very effective. It focuses on high consumption of healthy fats: As I explain in my book, "Fat for Fuel," you should aim for having 70 to 85 percent of your total daily calories to come from healthy fats. For your protein intake, the general rule of thumb is to follow the formula of 1 gram of protein for every kilogram of your lean body mass.
Collectively, these findings suggest that LDL particle size is a more precise indicator of future cardiovascular illness than total LDL cholesterol even when people have high levels of one but not the other. That being said, LDL-C is still a useful indicator for future cardiovascular illness and ideally you want to have both low LDL-C and LDL-P. In 2005, he would eventually die with his dream of academic acceptance unrealized. But my colleague Dr. Linda Isaacs and I have worked diligently over the past 26 years, keeping the Kelley idea alive, that different people may require completely different diets. In the next installment, I will address my own experience treating patients diagnosed with advanced cancer with a Kelley based approach. Our therapy involves, oftentimes, diets high in carbohydrates, which proponents of the ketogenic diet would predict should fuel, not stop, cancer.
Throughout his talks, both here and elsewhere, Dr. Seyfried presents mouse studies that are interesting and suggestive that there might be something to this whole ketogenic diet thing, at least in brain tumors, such as this one. However, this is what we in the oncology biz would call pretty preliminary data, worthy of further investigation but not supporting the grandiose claims that Dr. Seyfried makes.

In practice, a ketogenic diet is one that keeps net carbohydrates below 50 grams per day, although the absolute number necessary to achieve ketosis will vary from individual to individual and may be as high as 100 grams/day for some. 46 (Because fiber is indigestible when eaten, it is usually not included in the carbohydrate count on a ketogenic diet, therefore net carbohydrates equals the total amount of carbohydrate minus the fiber.) Including some protein and carbohydrate on a ketogenic diet is important too since it supplies the needed substrate for the body to produce ketones, but too much can interfere with ketosis.47 48 A ketogenic diet usually includes low-moderate amounts of meat, fish, poultry, and eggs, moderate amounts of low-carb vegetables such as leafy greens and broccoli, and lots of healthy fats like avocados, nuts, and seeds, coconut oil, olive oil, etc. For those intent on ensuring they are in ketosis, home devices for testing ketone levels are available.
Based on this logic, the keto diet appears to be a safe, inexpensive, easily implementable, and effective approach to selectively target cancer cells. Promisingly, multiple studies show that the keto diet reduces tumor growth and improves survival in animal models of multiple cancers, along with enhancing the effects of other forms of anticancer therapy.11,15 Similarly, fasting has been shown to enhance responsiveness to chemotherapy, along with reducing some of the side effects in preclinical cancer therapy models.11

When she first consulted with Dr. Kelley in 1977 she was in a near terminal state after having failed intensive chemotherapy. Nonetheless, despite her dire situation within a year she had experienced complete regression of her extensive bony lesions, as documented by x-ray studies. Though in subsequent years her compliance with her nutritional regimen would waver and her disease would in turn recur, invariably when she resumed Kelley’s treatment the myeloma would go into remission.
Typically known as the “bad cholesterol” to its healthy counterpart HDL cholesterol, increased levels of LDL cholesterol are associated with an increased risk of cardiovascular diseases (CVD). [14] Some studies show a strong correlation between LDL cholesterol and the risk of cardiovascular diseases in both men and women. [15] Evidence also suggests that decreasing blood levels of LDL-C reduces the risk of CVD. [16]
Over half of children who go on the diet have at least a 50% reduction in the number of their seizures. It can start to help after just one week, but more often it can take a few weeks up to several months to judge whether it will be effective. If the diet successfully controls seizures, it may be continued for several years under the supervision of the child's health care team. Many children on the ketogenic diet continue to take seizure medications but on average they require one less medication and often the dosage of remaining ones is lowered.

It seems to me that your’re presenting conflicting information. Your series had T. Colin Campbell and his research from the China Study. And the healing benefits of a plant based diet. Animal protein causes inflammation. Is there specific science on the ketogenic diet? I know people doing keto long term and they look awful and seem to be sick all the time. I have heard of the keto flu.

The ketogenic diet is indicated as an adjunctive (additional) treatment in children and young people with drug-resistant epilepsy.[26][27] It is approved by national clinical guidelines in Scotland,[27] England, and Wales[26] and reimbursed by nearly all US insurance companies.[28] Children with a focal lesion (a single point of brain abnormality causing the epilepsy) who would make suitable candidates for surgery are more likely to become seizure-free with surgery than with the ketogenic diet.[9][29] About a third of epilepsy centres that offer the ketogenic diet also offer a dietary therapy to adults. Some clinicians consider the two less restrictive dietary variants—the low glycaemic index treatment and the modified Atkins diet—to be more appropriate for adolescents and adults.[9] A liquid form of the ketogenic diet is particularly easy to prepare for, and well tolerated by, infants on formula and children who are tube-fed.[5][30]


The conventional view of cancer is that it is caused by DNA mutations in the cell nuclei. However, the metabolic theory of cancer proposes that some cancers are caused by a dysfunction of cellular respiration and that the restriction of glucose in the diet may prevent and even reverse some cancers. Today I’ll review the research supporting this theory and explore how the ketogenic diet may impact cancer tumor growth.
Of course, we know that genes alone are not responsible for cancer because we share many of the same genes as our hunter–gatherer ancestors and even just the same genes as our ancestors several generations ago, and yet the rate of cancer keeps going up. It’s expected to overtake cardiovascular disease as the number one cause of death in the U.S. fairly soon, and so that can’t be explained by genes alone.
“For me, the focus is on optimizing nutrition, and on nutritional interventions as part of the therapy for chronic diseases,” he said. “Depriving the body of access to any major macronutrient changes metabolism and invariably has a negative impact on health. The best nutritional intervention for weight loss is a caloric-restricted, portion-controlled, well-balanced meal plan.”

In fact, one study stated that “…the group with the highest risk for cardiovascular events had high LDL-P and LDL-C, while the group with the lowest risk had low LDL-P but higher LDL-C.” [17] As a result of prior clinical research and their findings, the researchers stated that: “While the low carb, ketogenic diet did not lower total LDL cholesterol, it did result in a shift from small, dense LDL to large, buoyant LDL, which could lower cardiovascular disease risk.” [23]
Editors responded with disbelief, claiming the results couldn’t be real since a non-toxic nutritional therapy could never be useful against advanced cancer. I found the logic, “it couldn’t be true because it couldn’t be true” perplexing, for editors of scientific journals. In any event, the book would finally be published, in a rewritten and updated form, in 2010.
Unfortunately, thanks to the widespread prevalence of the standard American diet (SAD) – characterized by high consumption of red meat, processed meat, pre-packaged foods, butter, fried foods, high-fat dairy products, refined grains, potatoes, corn, high-fructose corn syrup (HFCS), and high-sugar drinks – the incidence of obesity, insulin resistance, diabetes, cardiovascular disease, and many forms of cancer are skyrocketing in the U.S.
When there is ample glucose available (derived from dietary carbohydrates), the body will use that glucose as its main fuel source for producing ATP (the body’s main energy currency). Small amounts of glucose can be stored, but once incoming glucose has been used up and those stores run out, the body shifts gears and increases the use of fatty acids (derived from body fat stores or dietary fats) to produce energy. While most cells can directly use free fatty acids in order to produce sufficient energy, fats cannot cross the blood-brain barrier making them inaccessible to the brain as a fuel source. To overcome this, the liver converts fat into acetoacetate, acetone and beta-hydroxybutyrate (a.k.a. ketone bodies or ketones) which can be used by red blood cells and cells of the central nervous system (including the brain) as an alternative energy source when glucose levels are low. Importantly, the vast majority of other bodily tissues can also use ketones to produce energy. In fact, ketone production is a normal part of healthy metabolism and small amounts of ketones are produced regularly—such as during extended exercise or during an overnight fast. If glucose supplies continue to remain low, the body will increase its production of ketones. Using ketones as an alternative energy source reduces muscle protein breakdown, when carbohydrate is low. In addition to being used for energy, ketones have signaling functions that positively regulate genes related to aging, oxidative stress and healthy sympathetic nervous system activity. 6 7
Nonetheless, enthusiastic oncologists joined with the media, portraying insurance companies as heartless, greedy bullies depriving women with breast cancer of a curative treatment. Not too long after, the trial lawyers got involved, orchestrating a series of lawsuits against various insurance companies on behalf of women wanting a BMT. In a particularly notable and telling case, Fox vs. HealthNet, the jury awarded the plaintiff, a woman diagnosed with breast cancer whose insurance carrier refused to cover the procedure, $89 million, including $77 million in punitive damages.
All of this talk about energy metabolism leads us to another important finding in the cancer literature: healthy cells can also become cancerous if their mitochondria (the primary energy producer of the cell) is dysfunctional for an extended period of time. Furthermore, if the cell is already cancerous, the mitochondrial dysfunction will make the cell’s genes vulnerable to additional mutations.
Low-carbohydrate high fat diets (LCHF diets) consistently improve all other markers of cardiovascular risk — lowering elevated blood glucose, insulin, triglyceride, ApoB and saturated fat (especially palmitoleic acid) concentrations, reducing small dense LDL particle numbers, glycated haemoglobin (HbA1c) levels, blood pressure and body weight while increasing low HDL-cholesterol concentrations and reversing non-alcoholic fatty liver disease (NAFLD).
Collectively, these findings suggest that LDL particle size is a more precise indicator of future cardiovascular illness than total LDL cholesterol even when people have high levels of one but not the other. That being said, LDL-C is still a useful indicator for future cardiovascular illness and ideally you want to have both low LDL-C and LDL-P. In another parallel experiment the mice used did not have cancer at the start, but were bred to have a genetic predisposition toward breast cancer. Almost half of these mice, when fed on the Western diet, showed cancer within the first year (the average life span of these mice is two years). Only one of the mice in this group reached its normal life expectancy, and 70% ultimately died of cancer. Of the group on the ketogenic diet, only 30% ever developed cancer, and over half reached their normal life expectancy or exceeded it.
A recent systemic review and meta-analysis of randomized controlled trials comparing the long-term effects (greater than 1 year) of dietary interventions on weight loss showed no sound evidence for recommending low-fat diets. In fact, low-carbohydrate diets led to significantly greater weight loss compared to low-fat interventions. It was observed that a carbohydrate-restricted diet is better than a low-fat diet for retaining an individual’s BMR. In other words, the quality of calories consumed may affect the number of calories burned. BMR dropped by more than 400 kcal/day on a low-fat diet when compared to a very low-carb diet.
Despite the initial warning signs, the media continued its relentless promotion of interleukin-2 for a number of years. In 1992, perhaps due to political pressure more than scientific evidence, the FDA approved the drug for use against cancer, despite the lack of comprehensive controlled trials. Then in the late 1998 a clinical study – completed some 13 years after the initial reporting – showed that interleukin-2, at least with advanced kidney cancer, worked no better than placebo.
Uh, no. Not exactly. Preclinical experiments are intriguing but fairly limited in applicability, and the case reports demonstrate nothing of the sort. There’s more to Dr. Seyfried’s hypothesis, for example, his idea that metastatic cancer comes about because of alterations in glutamine metabolism, but unfortunately he appears to misunderstand the genetics of metastasis when he bases part of his conclusion on observations that metastatic cancers often have the same genetic derangements as the primary tumor. It’s been a longstanding question whether clones of tumor cells possess the ability to metastasize as an intrinsic part of the process of becoming cancer cells or whether they acquire it later. Given that evolution is a major force driving cancer cells to become more invasive and that tumors are very heterogeneous, full of lots of different clones with different sets of genetic mutations, Dr. Seyfried’s hypothesis is at best simplistic. Also disappointingly, the evidence for any diet as a treatment for cancer is weak at best.
Other concerns that may arise from remaining on a ketogenic diet long term include mineral deficiencies, decreased bone mass, decreased thyroid function, thinning hair and/or hair loss, heart problems, and menstrual irregularities.59 While it is possible that some of these problems can be mitigated with appropriate supplementation (e.g. fiber, adaptogenic herbs and minerals), ensuring regular adequate caloric intake, and adjusting carbohydrate intake to meet personal needs, not all people will thrive on a ketogenic diet. It is wise to work with an experienced practitioner who can help monitor blood work and symptoms for unwanted changes if you plan to stay on a ketogenic diet long-term.
