This diet is not without its side effects, so it is very important to become well informed when considering ketogenic diet therapy. Do not attempt the ketogenic diet without medical supervision from a properly trained ketogenic diet team, especially if you are taking anti-seizure medications. This ketogenic diet team includes a neurologist, a registered dietitian and nurse and sometimes a nurse practitioner, pharmacist, social worker and other specialists.
First of all, myeloma patients, even when diagnosed with an aggressive form, often linger for years before the disease advances. I would never have included such a two-year survivor in One Man Alone, or in any other book I have written or plan to write – unless, possibly, there has been documented significant regression of disease, not apparent in this case. I do include a case of multiple myeloma treated by Dr. Kelley in my monograph, a woman diagnosed with extensive cancer throughout her skeleton with evidence of multiple fractures.
"It's called ketogenic metabolic therapy," and he says in this context, "the ketogenic diet shouldn't be considered a diet like green salads or other such stuff. It's essentially medicine, and the process primarily tries to remove one of the driving fuels for the disease, which is glucose, and transition the whole body over to ketones, which the tumor cells can't use as a fuel."

Dr. Kossoff is a Professor of Neurology and Pediatrics at Johns Hopkins University in Baltimore, MD.  He received his medical degree from SUNY at Buffalo School of Medicine in New York, followed by a residency in pediatrics at Eastern Virginia Medical School in Norfolk, Virginia. He completed a fellowship in child neurology and then pediatric epilepsy and clinical neurophysiology at The Johns Hopkins Hospital in Baltimore.  He has been at Johns Hopkins since 1998.
The current body of research on metabolic cancer treatments using a keto diet continues to grow. Keto or metabolic cancer therapy is somewhat different than the treatment for other illnesses, and is discussed in detail in my book Fight Cancer with a Ketogenic Diet, which is based on the metabolic therapy research of Dr. Thomas Seyfried  and Dr. Dominic D'Agostino. The main idea behind the use of a ketogenic diet to treat cancer is to starve cancer cells of the sugar and other fuels they need to survive, and to provide support and protection for normal energy processes in healthy cells. The advantage of this treatment protocol is that it is non-toxic to the body, and in "wait and see" cases, it can provide an major health support advantage to the patient. In the case of Parkinson’s disease (PD) and Alzheimer disease (AD) models, there are data suggesting that calorie restriction itself is protective [31,32••], raising the question of whether manipulations of some critical metabolic pathways also may possess disease-modifying properties. The ketogenic diet originally was designed to mimic fasting, and thus it may regulate a family of proteins known as sirtuins, which play a major role in mediating “anti-aging” effects of calorie restriction [33•]. Alternatively, the ketogenic diet may regulate a master energy-sensing protein in the cell, 5′-adenosine monophosphate (AMP)–activated kinase [34]. Both proteins have a number of downstream effectors that may possess neuroprotective properties.

Animal research indicates that a ketogenic diet reduces levels of brain amyloid-beta, a misfolded protein that contributes to Alzheimer’s disease, while also restoring mitochondrial function and improving learning and memory. (18, 19, 20) Although fewer studies on a ketogenic diet have been done in humans with Alzheimer’s disease, a recent trial found a ketogenic diet to be both safe and effective for mild Alzheimer’s disease. (21)
To hit my fat targets with normal Filipino food, I used to mix coconut oil with my meats and veggies, and butter with my black coffee. Coconut oil and butter consists mostly of saturated fats. At the advice of one of my doctor friends, I’m currently experimenting with olive oil and peanuts, which are mostly monounsaturated fats, to hit the same fat targets. We’ll see how this affects my productivity and lipid profile.

What they didn’t realize was they were putting me at a greater risk for heart disease and they were putting me at a greater risk for having depression and all these kind of mental health issues. Your brain literally needs that cholesterol to run. If you’re not giving it the cholesterol in your diet, and you’re not allowing the natural progression of cholesterol to take place in the body, you’re actually causing harm to your body. This is the analogy I used in Cholesterol Clarity about the role that cholesterol plays in the body, so you say “What is the real problem that we should be paying attention to if it’s not cholesterol?” It’s the inflammation, stupid. Inflammation is really what we need to be paying attention to.
With this case study, it is important to note that patients with GBM rarely experience a rapid tumor regression after surgical resection and conventional therapy. Thus, the researchers emphasized that the “…response of the GBM in this patient after standard treatment alone would be unlikely, further suggesting a role for targeting energy metabolism as part of the management strategy.”
In addition to seizures, the KD has been suggested to treat neurodegenerative disorders (eg, Alzheimer’s, Parkinson’s), metabolic defects, migraines, and other disease states.3 Although the KD isn’t a mainstream form of treatment, the possibility that there may be clinical significance in using dietary means for certain medical conditions can’t be ruled out. This isn’t an endorsement for the KD as much as an expression of interest that, depending on nutritional intake, the body can make astounding adaptations to help control debilitating conditions.
The traditional Atkins’ Diet was certainly high fat, in the range of 70% or more, nearly all from animal sources, and with minimal dietary carbs, less than 10%. Dr. Atkins, famed for his all-encompassing emphasis on ketosis during his early years as a diet doctor, insisted his patients routinely check the levels of ketone bodies in their urine several times a day, using special “ketone strips.”
Everyone talks about upping their fats… I do not think that is the key to sweeping LDL out of the system. Upping cruciferous fiberous veggies… the fiber, vitamins and minerals contained in veggies bind with the LDL and move it on out. You would have to eat literally a truck load to make any serious dent in your daily carb allowance since most are very low net carb anyways.
The first modern study of fasting as a treatment for epilepsy was in France in 1911.[12] Twenty epilepsy patients of all ages were "detoxified" by consuming a low-calorie vegetarian diet, combined with periods of fasting and purging. Two benefited enormously, but most failed to maintain compliance with the imposed restrictions. The diet improved the patients' mental capabilities, in contrast to their medication, potassium bromide, which dulled the mind.[13]
[57:08] – While the experiment worked, in that it showed a decrease in lipid levels, other markers such as postprandial glucose levels increased. Dave talks about the importance of informing yourself, finding a balance and listening to how the body feels. He also talks about plans to continue experimenting with different levels but taking into account different risk-factors. Ultimately, he prefers a lower ketogenic ratio. Anecdotally, I’ve spoken with some cancer researchers who claim to be virtually curing cancer in animals using a combination of ketogenic diet and PI3K or mTOR, like rapamycin, but these data aren’t published. Again, we need to be cautious about accepting these claims until they’ve gone through the legitimate scientific channels and people have had a chance to review this research.
This device measures the amount of acetone (another ketone) you release in your breath when you are in ketosis. (Acetone is also the ketone responsible for bad breath when you’re in ketosis.) There are a number of kits on the market. One study showed they are as effective as ketone strips for testing ketone levels, but critics argue both have drawbacks including testing for only one specific ketone.
To determine whether you’re in ketosis and what degree of ketosis you’re in, test your ketones each morning. Blood ketone testing is the most accurate method—I do not recommend breath or urine ketone monitoring. In our practice, we recommend the Precision Xtra Blood Glucose Meter Kit, which can be purchased on Amazon (you can buy test strips for this meter in bulk on eBay for a lower cost). Keto Mojo is another good meter with affordable test strips.
Eliminating several food groups and the potential for unpleasant symptoms may make compliance difficult. An emphasis on foods high in saturated fat also counters recommendations from the Dietary Guidelines for Americans and the American Heart Association and may have adverse effects on blood LDL cholesterol. However, it is possible to modify the diet to emphasize foods low in saturated fat such as olive oil, avocado, nuts, seeds, and fatty fish.

In one study, a variant of the ketogenic diet was applied to children with autism [51, Class III]. This diet was a modified John Radcliffe diet, which substitutes medium-chain triglycerides for some fat, but it was administered for only 4 of every 6 weeks during this 6-month trial (ie, cycles of 4 weeks “on diet” and 2 weeks “off diet” were used for the duration of the study). This group studied children on Crete, an island with a relatively isolated population and a significant number of autistic children. Behavior was rated on the standardized Childhood Autism Rating Scale (CARS) by a blinded child psychiatrist. Of the 18 children who completed the study, 2 demonstrated significant improvement (ie, CARS score reduced by > 12 points), 8 had moderate improvement (CARS score reduced by 8–12 points), and 8 showed minor improvement (CARS score reduced by 2–8 points). Children with lower starting CARS scores (less severe autism) appeared to respond better than those more severely affected. These findings should be interpreted with caution for a number of reasons. Given the geographic isolation of Crete, there may have been a strong genetic contribution to autism in this population. Methodologically, the CARS score was not designed as a longitudinal test, making its meaning in this study unclear. Additionally, intermittent administration of the ketogenic diet has not been examined in other disorders, making it difficult to compare this intervention with other studies of the ketogenic diet. Finally, any structured intervention may be associated with improved performance in patients with autism. Further study with appropriate controls (structured diet plans, vitamin administration) is needed to confirm these findings.
Use fat as a lever.  We’ve been taught to fear fat, but don’t! Both keto and low carb are high fat diets. Fat is our source of energy as well as satiety. The key to understand, though, is that fat is a lever on a low carb or keto diet. Carbs and protein stay constant, and fat is the one you increase or decrease (push the lever up or down) to gain or lose weight, respectively. So if your goal is weight loss, eat enough fat to be satisfied, but there’s no need to “get your fats in” once you’re satisfied.
By the way, the relative heaviness of stored glycogen is why many people report fast weight loss on a ketogenic or low-carb diet: their body has dumped a little extra weight in the form of glycogen and water (which tags along with glycogen in a 3 parts water to 1 part glycogen ratio). Unfortunately, this water and glycogen comes right back once we start eating normally again.
Ive been strict keto for over 3 months and have lost 20lbs. I got my test results back yesterday and was shocked to see how my levels had changed: total cholesterol went from 230 to 308!! All the bad went up and all the good went down. But I’ve also heard that it can take a bit longer for cholesterol levels to even out and start going down. Is this true? I don’t want to quit keto because I have another 20lbs to lose, but I don’t want to have a stroke either.
Dr. Jockers, thank you so much for this clear and detailed article! I began a keto-style diet around August 2019. By late November, I had fallen from 197 lbs. to under 175 lbs., dropped from 28% to 18% body fat, and anecdotally felt much better in all aspects of my health. All of this occurred exclusively due to diet, I believe– I had almost no exercise routine to speak of, and my desk-based job is pretty sedentary. My family practice doc was surprised and happy with the results as well when I met with him in January… until my blood work came back showing total cholesterol at 257, triglycerides at 236, LDL-C at 162, and HDL at 50.

"This is something that physicians will want to include in their discussion about the risks and benefits of this particular treatment," the director of Detroit's Henry Ford Comprehensive Epilepsy Program tells WebMD. "But we know that uncontrolled seizures carry all kinds of risks. This remains a useful treatment. But like many treatments, it is not without risks."


Cancer cells need to carefully maintain their “redox status”. Redox status is the balance between oxidants and antioxidants. Oxidants, including free radicals and other “reactive” chemical species, are made continuously in every living cell as a byproduct of metabolic activities. Several antioxidant systems have evolved in our body to specifically counter the harmful actions of these oxidants.

Perhaps the first person to really put ketogenic diets on the map was Dr. Robert C. Atkins, a cantankerous medical doctor who began experimenting with a low-carb diet in 1963. His first book, Dr. Atkins Diet Revolution, was published in 1972. Dr. Atkins died in 2003 from a tragic fall (interestingly, the Atkins website’s timeline does not mention his death), but his diet plan lives on today.


Insulin is a hormone that lets your body use or store sugar as fuel. Ketogenic diets make you burn through this fuel quickly, so you don’t need to store it. This means your body needs -- and makes -- less insulin. Those lower levels may help protect you against some kinds of cancer or even slow the growth of cancer cells. More research is needed on this, though.
After the 12 weeks intervention, researchers noted that the concentration of LDL particles decreased by 9.6% from 1180 nmol/L 1180 to 1066 nmol/L. [22] As previously mentioned, lower levels of LDL particles are beneficial to cardiovascular fitness. Additionally, the particle size of LDL increased by an average of 5.2% from 20.75 mm to 21.27 mm. [22]
The ketogenic diet, or even just a higher-fat, low-carb diet, has now gained massive support as a modern-day healing strategy. In fact, our ideas about fats and cholesterol seem to have been almost completely reversed in recent years. That being said, many people see their cholesterol go up after beginning more of a ketogenic lifestyle. Some people get concerned when this happens, so in this article, I am going to address the phenomena of high cholesterol on a ketogenic diet.
There are other studies, but little or nothing in the way of randomized clinical trials. For instance, a recent retrospective study of 53 patients, of whom only six followed a ketogenic diet while being treated for GBM, concluded that the diet was safe, but no suggestion of efficacy was noted. More recently, a German group examined the effect of a ketogenic diet on 16 patients with advanced cancer of various types who had exhausted all therapeutic options. The treatment didn’t result in any serious side effects, although subjects found it very difficult to maintain the diet, particularly in the context of family life. Only five were able to complete the three month treatment period, and it was reported that these five didn’t have progression while on the diet. Of the remaining 11, two died early, one was unable to tolerate the diet and dropped out very quickly, two dropped out for personal reasons, one couldn’t continue the diet for more than a month and three had disease progression within less than 2 months of starting the diet and one dropped out to resume chemotherapy. As a whole, this study was well-nigh uninterpretable due to the different kinds of cancer, other than to conclude that less than 50% of patients with advanced cancer could adhere to the diet, and that those who could generally had no significant side effects. Of course, it’s unclear whether the diet helped the five who could adhere to it or whether those who adhered to it could do so because they had more indolent, less aggressive disease.
The weight and body mass index of the patients decreased significantly (P<0.0001). The level of total cholesterol decreased from week 1 to week 24. HDL cholesterol levels significantly increased, whereas LDL cholesterol levels significantly decreased after treatment. The level of triglycerides decreased significantly following 24 weeks of treatment. The level of blood glucose significantly decreased. The changes in the level of urea and creatinine were not statistically significant.
You can get cholesterol from eating animal foods like eggs, cheese, meat, and dairy but your body can also produce its own. An average 150 lb weighing male can synthesize 1000 mg of cholesterol a day. One single egg has 200 mg of cholesterol. The typical US dietary intake of cholesterol is about 307 mg[iv]. In that case, about 75% of your body’s cholesterol gets produced by the body internally and 25% gets ingested externally[v].
I enjoyed reading this book. The book takes you from the fact that cancer cells addict glucose to why and how ketogenic diet (low carbohydrate high fat diet) can help to fight and cure cancer. It also helps patients to customize their diet and provides cooking techniques. As I lost my uncle due to cancer, I felt Ellen Davis words "I wish with all my heart that I had known then what I know now"
Additionally, HDL cholesterol may have anti-inflammatory effects. A recent research study published by De Nardo et al shows that HDL may be responsible in reducing inflammatory activity by regulating immune system cells called macrophages. [5] Furthermore, epidemiological studies have noted an inverse association between levels of HDL and certain forms of cancer. [6, 7]

Collectively, these findings suggest that LDL particle size is a more precise indicator of future cardiovascular illness than total LDL cholesterol even when people have high levels of one but not the other. That being said, LDL-C is still a useful indicator for future cardiovascular illness and ideally you want to have both low LDL-C and LDL-P. Now a report, appearing several weeks ago in the journal Neurology, reveals that in fact, a ketogenic diet is also profoundly helpful in adults as well in terms of treating epilepsy. This research, published by investigators in Maryland, found that there was at least a 50% reduction in seizures in 32% of patients treated with a ketogenic diet as well as in 29% of patients who went on a modified Atkins diet. In fact, 9% of those placed on the ketogenic diet and 5% of those placed on the modified Atkins diet had a greater than 90% reduction in the frequency of their epileptic seizures. These diets were designed such that the bulk of calories, between 67% and 75%, came from fat. The study revealed that “the anticonvulsant effect occurs quickly with both diets, within days to weeks.” Interestingly, the most common side effect was weight-loss which the office indicated “maybe advantageous inpatients with obesity.”
Similar trends in adverse events have been noted in children. One key concern is the high risk of osteoporosis, which reaches as high as 20% in KD cases. Along with osteoporosis come growth issues. “Children on the classic KD do often grow slightly slower than children not on the diet. When the diet stops, there is ‘catch-up’ growth, usually to normal,” said Kossoff.
An overwhelming majority (90%) of parents said that they would. Even though the keto diet is extremely restrictive, time consuming, and requires rigid maintenance, most parents found the potential benefits outweighed its drawbacks. Many parents in the study were more concerned about the side effects of the medications―and were grateful for the opportunity to explore an alternative option. Further, 55% would consider trying the diet again.
The classic ketogenic diet is not a balanced diet and only contains tiny portions of fresh fruit and vegetables, fortified cereals, and calcium-rich foods. In particular, the B vitamins, calcium, and vitamin D must be artificially supplemented. This is achieved by taking two sugar-free supplements designed for the patient's age: a multivitamin with minerals and calcium with vitamin D.[18] A typical day of food for a child on a 4:1 ratio, 1,500 kcal (6,300 kJ) ketogenic diet comprises three small meals and three small snacks:[28]

We are brazilian, living in Brazil. My daughter, Isabel, 21y. o., born in 1996, has syndrome of deficiency of Glut1. She was diagnosed around her first year of life. At that time her baby bottle, her begining diet meal, was 50ml water plus 50ml oil plus vitamin. Since then, which means, for 20 years, she is under this diet. For almost 18 years under 4:1 proportion. At this right moment 3:1. The only problem she had since started the diet were kidney stones in 2002. Nothing else. Grateful to the diet she doesn’t take any kind of medicine to avoid seizures. Her health is perfect, no colesterol at all. We are at your will for any issues related to her health.
This describes me, too. I am not a doctor, but after months and years of research, have decided to follow the ketogenic lifestyle and the naturopathic cancer treatments prescribed by my physician. It took a few months to find a doctor who embraced this, but he is worth it. Keto had my diabetes under control long before we realized that cancer was trying to get me as well. I take no chemical drugs for anything. Diet, exercise, and a few supplements take care of my health needs. You don’t know me, but three years ago, I needed a walker and a wheelchair. Today at 60, I walk under my own power and ride a bicycle as much as possible. Go as natural as you can, it helps. Stay away from “products” and just eat fresh, whole, natural food. In practice, a ketogenic diet is one that keeps net carbohydrates below 50 grams per day, although the absolute number necessary to achieve ketosis will vary from individual to individual and may be as high as 100 grams/day for some. 46 (Because fiber is indigestible when eaten, it is usually not included in the carbohydrate count on a ketogenic diet, therefore net carbohydrates equals the total amount of carbohydrate minus the fiber.) Including some protein and carbohydrate on a ketogenic diet is important too since it supplies the needed substrate for the body to produce ketones, but too much can interfere with ketosis.47 48 A ketogenic diet usually includes low-moderate amounts of meat, fish, poultry, and eggs, moderate amounts of low-carb vegetables such as leafy greens and broccoli, and lots of healthy fats like avocados, nuts, and seeds, coconut oil, olive oil, etc. For those intent on ensuring they are in ketosis, home devices for testing ketone levels are available.
In this same chapter, there are also two case reports, neither very impressive. The first, written by the mother, tells the story of a four-year old child diagnosed in 2004 with a low-grade (less aggressive) but quite large and inoperable brain tumor. The parents, as the mother writes, entrusted their child into the hands of the experts, who prescribed the usual “gold standard” treatments, which are not clearly described initially but presumably mean chemotherapy and perhaps radiation.
The ketogenic diet is a mainstream dietary therapy that was developed to reproduce the success and remove the limitations of the non-mainstream use of fasting to treat epilepsy.[Note 2] Although popular in the 1920s and '30s, it was largely abandoned in favour of new anticonvulsant drugs.[1] Most individuals with epilepsy can successfully control their seizures with medication. However, 20–30% fail to achieve such control despite trying a number of different drugs.[9] For this group, and for children in particular, the diet has once again found a role in epilepsy management.[1][10]
To counteract these genetic vulnerabilities, it is may be best to eat a low to moderate fat diet that is whole food based with plenty of fiber, monounsaturated fats, polyunsaturated fats (especially omega 3s), and limited saturated fats. This, as well as a lifestyle filled with physical activity, stress relief practices, and plenty of sleep, should keep their cholesterol levels under control.
Having tempting, unhealthy foods in your home is one of the biggest reasons for failure when starting any diet. To maximize your chances of success with the keto diet, you need to remove as many triggers as you can. This crucial step will help prevent moments of weakness from ruining all your hard work.If you aren’t living alone, make sure to discuss with your family or housemates before throwing anything out. If some items are simply not yours to throw out, try to compromise and agree on a special location so you can keep them out of sight.
Ketone bodies, acetoacetate, and β-hydroxybutyrate (βOHB), are byproducts of fatty acid oxidation in the mitochondrial matrix of the hepatocytes. There are many theories about the role of KB, but the existence of an anticonvulsant effect is controversial. Some authors have found no relationship between KB and synaptic transmission and seizure control.
I wanted to put it out there that I made this meal plan specifically with women in mind. I took an average of about 150 women and what their macros were. The end result was 1600 calories – broken down into 136g of fat, 74g of protein, and 20g net carbs a day. This is all built around a sedentary lifestyle, like most of us live. If you need to increase or decrease calories, you will need to do that on your own terms.
Ketosis is a natural metabolic state where the body uses fat as its primary fuel source. In our studies and others, the ketogenic diet has shown to significantly lower blood glucose. This can trigger anti-inflammatory processes that enhance cellular function and improve overall dog health. The ketogenic diet, also known as the traditional ketogenic diet and modified Atkin’s diet, is an important and validated dietary approach to controlling intractable epilepsy that focuses on a high-fat, protein, and low-carb diet. Yet despite its success in cases of drug-resistant epilepsy, the mechanism behind its effectiveness is still not well understood.  
For most people, the keto diet will result in weight loss, but this might not be the healthiest way to do it. When your body burns fat because it is starved of carbs, it makes ketones. Ketones are a type of acid made by your liver and then sent into your bloodstream. Too many ketones can led to dehydration and alter the chemical balance of your blood.
To get the most benefit from the Keto diet, you should stay physically active. You might need to take it easier during the early ketosis period, especially if you feel fatigued or lightheaded. Walking, running, doing aerobics, weightlifting, training with kettlebells or whatever workout you prefer will boost your energy further. You can find books and online resources on how to adapt Keto meals or snacks for athletic training.
The total-to-HDL cholesterol ratio is found by dividing your total cholesterol level by your HDL-C, and it is essentially the same thing as an LDL-to-HDL cholesterol ratio since most non-HDL cholesterol is LDL cholesterol [26]. The researchers of the 2003 meta-analysis used this ratio because it is a better cardiovascular risk predictor than total cholesterol levels [25].
Once the body is adapted to ketosis, constipation and/or diarrhea are the most commonly reported side effects along with increased urination. Continuing to keep your mineral intake high and ensuring adequate water and fiber intake will help to counter these effects. People in ketosis may also notice a sweet or fruity odor on their breath, which is the result of increased production of the ketone acetone, which is a very volatile compound that is eliminated mainly through respiration in the lungs. 50
Spices are an easy way of adding more flavor, vitamins and antioxidants into your food. Furthermore, they are low in carbohydrates. Make sure that you're using fresh, organic spices for maximum flavor and nutrients. Some spices sold in packets found at the local grocery should not be used, as they often contain fillers that can increase your carbohydrate consumption, thus putting you out of ketosis.26

We have solid evidence showing that a ketogenic diet reduces seizures in children, sometimes as effectively as medication. Because of these neuroprotective effects, questions have been raised about the possible benefits for other brain disorders such as Parkinson’s, Alzheimer’s, multiple sclerosis, sleep disorders, autism, and even brain cancer. However, there are no human studies to support recommending ketosis to treat these conditions.
On the other hand, higher HDL cholesterol levels have been linked to less carotid artery intima-media thickness. In a large meta-analysis of data from more than 20,000 people, CIMT tended to decrease as HDL cholesterol increased – regardless of LDL cholesterol values (16). Importantly, although LDL response to carb restriction varies from person to person, HDL virtually always increases.
Check the nutrition labels on all your products to see if they’re high in carbs. There are hidden carbs in the unlikeliest of places (like ketchup and canned soups). Try to avoid buying products with dozens of incomprehensible ingredients. Less is usually healthier.Always check the serving sizes against the carb counts. Manufacturers can sometimes recommend inconceivably small serving sizes to seemingly reduce calorie and carb numbers.
