I’ve been Keto for over a year now. I’ve lost 60 lbs. I’m off Metformin for type 2 diabetic and my LDL has improved significantly. In my experience, Keto will show some quick results in some but for others it will take longer. It depends on how long you’ve been on the standard American diet. It will take time for your body to fix “self-heal”. Not everyone will have the same results in the same amount of time.
I have been on a low carb keto diet for more than a year. As T2DM my A1C dropped from 9% to 5.4% & I discontinued meds. All my lipids improved even with ample healthy saturated fat. More than a year now so I wonder why this would be a short term improvement when its obvious that I will not go back to a high A1C and taking 3 diabetes medications including sulphonylureas. It is clear from this article that you lack the necessary experience that would be gained from wholeheartedly trying the diet or monitoring patients doing it properly like me. I would be probably be facing my first amputation if I believed the negativity in your article. So for people with diabetes who may be dissuaded by your article. Ignore it and take back your health by restricting carbs (<25 g a day) or as low as you reasonably can below 130g while being satisfied that you are getting adequate nutrition.
Ketone production is the body’s natural response to very low or no carbohydrate availability. The ketogenic diet was developed to mimic the effects of fasting while still supplying adequate nutrition to maintain health. The classic ketogenic diet, developed in the early 1900s for children with epilepsy, consisted of a 3 or 4:1 ratio of fats to protein and carbohydrates combined, supplying up to 90% of the daily calories from fats.44 More recently, modified versions have emerged with similar rates of efficacy, and the wider variety of food and flexibility make the diet less arduous. Two such possible modifications are: a 1:1 and 2:1 ratios of fats to combined protein and carbohydrates and/or the addition of medium chain triglyceride (MCT) oil supplements. The medium chain fats that makeup MCTs more readily produce ketones than the long chain fats commonly found in the diet, allowing for a lower amount of total fat intake, and therefore increasing the amount of carbohydrate and protein that can be included in the diet.45
For many years, LDL-C tests have been used as the primary method of measuring LDL in the blood. It is cheaper and easier to measure. I just got to know too that my Cholesterol raised a lot, I've been on keto for a little more than 3 months and before that always had a low carb diet. I don't know if the 3 day fast I did before taking the blood test only " aggravated" the situation. The total is 302, LDL is 214! But since the triglycerides are 94 and HDL 57, it seems to be OK. I read that a low rate of TG/HDL may indicate that the LDL particles are of the bigger, fluffier type less dangerous.

The ketogenic diet is proposed as a potential adjuvant therapy by exploiting these differences between cancer and normal cells. Consuming a ketogenic diet reduces blood glucose levels through a drastic reduction in the amount of carbohydrates consumed.1,2 As a result of decreased blood glucose levels, less insulin is secreted, which downregulates signaling pathways that are frequently constitutively active in tumor cells.2 Because glucose metabolism is inhibited, energy must be primarily derived from fats.1 Fat metabolism results in the production of ketone bodies and β-hydroxybutyrate by the liver, which are used to fuel energy production. Cancer cells have difficulty using these pathways because they rely on glucose; the metabolism of fat increases oxidative stress. 

Seyfried agrees that there is clear evidence that cancer is a genetic disease, since we can inherit mutations that are clearly associated with increased cancer risk. That’s not at all controversial. That’s well established, and even Seyfried agrees with that. But he argues that many of these mutations that we can inherit are mutations that actually disturb cellular respiration, maybe that the heritable aspect of cancer is not mutation that drives itself—cellular proliferation—but instead are mutations that actually cause mitochondrial dysfunction and defects in cellular respiration. He also points out that many of the non-inherited causes of cancer that have been identified and are clearly recognized, like radiation, impair mitochondrial function. That may be a common mechanism that is shared between these non-inherited causes of cancer and inherited causes of cancer.
He followed her advice, refused radiation, came to see me, and over the years he has proven to be a very vigilant, determined and compliant patient. High-protein ketogenic diet (HPKD): This version of the keto diet is often followed by folks who want to preserve their muscle mass like bodybuilders and older people. Rather than protein making up 20 percent of the diet, here it’s 30 percent. Meanwhile, fat goes down to 65 percent of the diet and carbs stay at 5 percent. (Caution: folks with kidney issues shouldn’t up their protein too much.)

A growing body of research indicates that ketogenic diets are more effective than low-fat diets for sustaining long-term weight loss in obese adults. (2, 3) Furthermore, the weight loss observed on a ketogenic diet is primarily visceral fat, the hard-to-lose fat located deep in the abdominal cavity; lean body mass, on the other hand, is preserved. So guess what your body will do when you increase the dietary cholesterol…that’s right, it’ll stop producing its own cholesterol. Since 75% of your cholesterol is made inside your body, if you increase the amount that you eat, your body will cut that 75% down. Your body isn’t stupid. It knows what it needs.
Available research on the ketogenic diet for weight loss is still limited. Most of the studies so far have had a small number of participants, were short-term (12 weeks or less), and did not include control groups. Some would argue, however, that these cut-off points are arbitrary and do not apply to all individuals depending on their baseline metabolic health and overall health. For instance, far too many examples exist of people with low LDL levels having heart attacks and those with high LDL having improved longevity. Selecting the right food will be easier as you become accustomed to the Keto approach. Instead of lean meats, you’ll focus on skin-on poultry, fattier parts like chicken thighs, rib-eye steaks, grass-fed ground beef, fattier fish like salmon, beef brisket or pork shoulder, and bacon. In his books and in his office working with his own patients, Dr. Atkins warned that to reap the benefits of his diet, one must reach and stay in a state of ketosis, much like the traditional Eskimos. Even a slight deviation from the diet, some ill-advised cheating with a cookie or candy, could stop ketosis in its tracks, and with it, the value of the diet.
Physicians of ancient Greece treated diseases, including epilepsy, by altering their patients' diet. An early treatise in the Hippocratic Corpus, On the Sacred Disease, covers the disease; it dates from c. 400 BC. Its author argued against the prevailing view that epilepsy was supernatural in origin and cure, and proposed that dietary therapy had a rational and physical basis.[Note 3] In the same collection, the author of Epidemics describes the case of a man whose epilepsy is cured as quickly as it had appeared, through complete abstinence of food and drink.[Note 4] The royal physician Erasistratus declared, "One inclining to epilepsy should be made to fast without mercy and be put on short rations."[Note 5] Galen believed an "attenuating diet"[Note 6] might afford a cure in mild cases and be helpful in others.[11]
“Net carbs” and “impact carbs” are familiar phrases in ketogenic diets as well as diabetic diets. They are unregulated interchangeable terms invented by food manufacturers as a marketing strategy, appearing on some food labels to claim that the product contains less “usable” carbohydrate than is listed. [6] Net carbs or impact carbs are the amount of carbohydrate that are directly absorbed by the body and contribute calories. They are calculated by subtracting the amount of indigestible carbohydrates from the total carbohydrate amount. Indigestible (unabsorbed) carbohydrates include insoluble fibers from whole grains, fruits, and vegetables; and sugar alcohols, such as mannitol, sorbitol, and xylitol commonly used in sugar-free diabetic food products. However, these calculations are not an exact or reliable science because the effect of sugar alcohols on absorption and blood sugar can vary. Some sugar alcohols may still contribute calories and raise blood sugar. The total calorie level also does not change despite the amount of net carbs, which is an important factor with weight loss. There is debate even within the ketogenic diet community about the value of using net carbs.
A: The most common ways to track your carbs is through MyFitnessPal and their mobile app. You cannot track net carbs on the app, although you can track your total carb intake and your total fiber intake. Cancer cells need to carefully maintain their “redox status”. Redox status is the balance between oxidants and antioxidants. Oxidants, including free radicals and other “reactive” chemical species, are made continuously in every living cell as a byproduct of metabolic activities. Several antioxidant systems have evolved in our body to specifically counter the harmful actions of these oxidants.

There are three instances where there’s research to back up a ketogenic diet, including to help control type 2 diabetes, as part of epilepsy treatment, or for weight loss, says Mattinson. “In terms of diabetes, there is some promising research showing that the ketogenic diet may improve glycemic control. It may cause a reduction in A1C — a key test for diabetes that measures a person’s average blood sugar control over two to three months — something that may help you reduce medication use,” she says.
Cholesterol is a type of molecule known as a “steroid alcohol.” Every cell in your body needs cholesterol. 75% of all your average required cholesterol is produced by the body. The rest (25%) is consumed through food. Only animals use cholesterol. The average human body contains roughly 30 to 40 grams of cholesterol, most of which resides in cell membranes. Pretty much every cell can make cholesterol.

Sleep enough – for most people at least seven hours per night on average – and keep stress under control. Sleep deprivation and stress hormones raise blood sugar levels, slowing ketosis and weight loss a bit. Plus they might make it harder to stick to a keto diet, and resist temptations. So while handling sleep and stress will not get you into ketosis on it’s own, it’s still worth thinking about.
Leanne: Billion. Okay, let’s talk a little bit about Staten, I can’t remember what book it was but they were saying that cholesterol is really important for brain health. You mentioned that too, the cholesterol uses the nutrient and moves it up to your brain, it’s really important. What I was reading is that as we age our cholesterol actually increases slightly to help with that aging process. You often meet people that are in their 40s, 50s, I know a lot of people in my family are now on Staten because their cholesterol is increasing. I always say, “Maybe that’s your body’s way of protecting yourself against aging.”
This was a great read. I aim to restrict carbs always because I believe most are why the American population is obese. I would very much like to hear more about carb restriction excluding the discussion on processed meats and processed high salt content foods because I consume neither. I also don’t consume dairy or eggs. So can you provide some substance.
In its strictest form, the ketogenic diet provides more than 90 percent of its calories through fat (as compared to the 25 to 40 percent usually recommended for children). When we burn fat for energy, rather than glucose from carbohydrates, we produce compounds known as ketone bodies—hence the name “ketogenic diet.” The increase in ketones—referred to as ketosis—is thought to have an anticonvulsant effect in the brain, although how this works is still something of a mystery.
“Your liver produces ketones all the time, but the rate depends on carbohydrate and protein intake,” says Jeff Volek, Ph.D., R.D., a professor of human sciences at Ohio State University. When the majority of your diet is made up of of carbs and protein, ketogenesis slows. Replacing carbs and protein with fat will put your body into ketosis, thus ramping up ketone production. This takes about three days to induce.
A great deal of exciting research is emerging regarding the application of a ketogenic diet in the treatment of neurodegenerative diseases, including Alzheimer’s and Parkinson’s disease. (17) Neurodegenerative diseases are characterized by brain insulin resistance, a condition that starves neurons of the glucose they normally need to function correctly. Scientists have found that ketones are an excellent alternative fuel for the insulin-resistant brain. In addition, ketones reduce brain oxidative stress and mitochondrial dysfunction, two significant factors in the neurodegenerative disease process.
You want to keep your cheats to none. Be prepared, make sure you’re eating what you need to be satiated (“full”), and make sure you’re satisfied with what you’re eating. If you have to force yourself to eat something, it will never work out in the end. This is just a guideline on how you can eat on a ketogenic diet, so you’re very welcome to change up what kind of foods you eat!
The Ketogenic Diet (KD), a high-fat/low-carbohydrate/adequate-protein diet, has recently been proposed as an adjuvant therapy in cancer treatment [1]. KDs target the Warburg effect, a biochemical phenomenon in which cancer cells predominantly utilize glycolysis instead of oxidative phosphorylation to produce ATP. Furthermore, some cancers lack the ability to metabolize ketone bodies, due to mitochondrial dysfunction and down-regulation of enzymes necessary for ketone utilization [2]. Thus, the rationale in providing a fat-rich, low-carbohydrate diet in cancer therapy is to reduce circulating glucose levels and induce ketosis such that cancer cells are starved of energy while normal cells adapt their metabolism to use ketone bodies and survive. Furthermore, by reducing blood glucose also levels of insulin and insulin-like growth factor, which are important drivers of cancer cell proliferation, drop.
Contemporary researchers like Dr. Thomas Seyfried and Dominic D’Agostino have argued that this dysregulated cellular energy production, or cellular metabolism, is actually what induces malignancy and that by extension, if we limit the fuels available for this process of fermentation, and the fuels are glucose, which is derived from carbohydrate in the diet, and glutamine, which is derived from protein in the diet, then we can actually starve cancer cells and either improve the results of conventional treatment or perhaps even address some cancers independently without conventional treatment.

Jimmy Moore: That’s a loaded question and we could probably go 3 hours just on that but I’ll give you the nut shell. Most medical professionals have been taught to look at 2 primary numbers on your cholesterol panel. Total cholesterol, and this number called LDLC, which is your LDL cholesterol that you’ve heard about. You’ve heard LDL is bad and you’ve heard if you’re total cholesterol level is over 200, oh my god you are at great risk for heart disease. Exactly.


The ketogenic diet is calculated by a dietitian for each child. Age, weight, activity levels, culture, and food preferences all affect the meal plan. First, the energy requirements are set at 80–90% of the recommended daily amounts (RDA) for the child's age (the high-fat diet requires less energy to process than a typical high-carbohydrate diet). Highly active children or those with muscle spasticity require more food energy than this; immobile children require less. The ketogenic ratio of the diet compares the weight of fat to the combined weight of carbohydrate and protein. This is typically 4:1, but children who are younger than 18 months, older than 12 years, or who are obese may be started on a 3:1 ratio. Fat is energy-rich, with 9 kcal/g (38 kJ/g) compared to 4 kcal/g (17 kJ/g) for carbohydrate or protein, so portions on the ketogenic diet are smaller than normal. The quantity of fat in the diet can be calculated from the overall energy requirements and the chosen ketogenic ratio. Next, the protein levels are set to allow for growth and body maintenance, and are around 1 g protein for each kg of body weight. Lastly, the amount of carbohydrate is set according to what allowance is left while maintaining the chosen ratio. Any carbohydrate in medications or supplements must be subtracted from this allowance. The total daily amount of fat, protein, and carbohydrate is then evenly divided across the meals.[37]
The study also found that two types of bacteria, called Akkermansia muciniphila and Parabacteroides, were elevated by the diet. When these two types of bacteria were given in combination to mice that didn't have their own gut bacteria, the anti-seizure effect of the keto diet was restored. What's more, this combination of bacteria protected against seizures even if the mice were fed a nno-keto diet.

In 1921, Rollin Turner Woodyatt reviewed the research on diet and diabetes. He reported that three water-soluble compounds, β-hydroxybutyrate, acetoacetate, and acetone (known collectively as ketone bodies), were produced by the liver in otherwise healthy people when they were starved or if they consumed a very low-carbohydrate, high-fat diet.[10] Dr. Russell Morse Wilder, at the Mayo Clinic, built on this research and coined the term "ketogenic diet" to describe a diet that produced a high level of ketone bodies in the blood (ketonemia) through an excess of fat and lack of carbohydrate. Wilder hoped to obtain the benefits of fasting in a dietary therapy that could be maintained indefinitely. His trial on a few epilepsy patients in 1921 was the first use of the ketogenic diet as a treatment for epilepsy.[10]
The second is called LDL-P which measures the number of LDL particles in the blood. Sometimes, there is a correlation – more LDL particles means that you can have higher levels of LDL-C. However, larger LDL molecules can grow and carry more cholesterol – leading to a discordance in which LDL-C and LDL-P are not necessarily proportional. When this happens, LDL-C and LDL-P are said to be “discordant.”

Agreed. The big problem is that a person can't (or shouldn't) get a CACS often enough to prove that a dietary change will make a difference. Calcification is usually slow & the radiation from the scan isn't trivial. Medical professionals or patients can jump to a conclusion & remedy/change their diets immediately. I would go with the person's instincts (do you feel better on this diet?) & get the appropriate scanning tests done after a sizeable interval.  
The ketogenic diet is usually initiated in combination with the patient's existing anticonvulsant regimen, though patients may be weaned off anticonvulsants if the diet is successful. Some evidence of synergistic benefits is seen when the diet is combined with the vagus nerve stimulator or with the drug zonisamide, and that the diet may be less successful in children receiving phenobarbital.[18]
Familial hypercholesterolemia is a condition where one or more of the genes for the LDL receptor are defective. Having the right amount of properly functioning LDL receptors plays a vital role in maintaining healthy cholesterol levels by clearing LDL cholesterol out of the blood. Without an adequate amount of LDL receptors, the blood is much more likely to accumulate high levels of LDL particles, increasing the body’s vulnerability to cardiovascular disease significantly. [28]

On a “normal” American diet, carbohydrate intake is high (about 40-60% of calories) while fat intake, and especially saturated fat, is limited. In contrast, carbohydrate intake on a keto diet is only about 2-4% of calories. When carb intake is low, meals are delicious and satiating. Hunger goes away, and more importantly, this dietary change has some powerful and beneficial metabolic effects on the human body, in part because it lowers blood sugar and insulin levels. 
We admit approximately four children ranging from infants to adolescents each month to participate in the therapeutic ketogenic diet program. New patients take part in a 3-day orientation (Monday through Wednesday) that starts the child on the diet and provides education for the family. After that, we follow up with the patients in our clinic every one to three months.

If you're new to keto, watch out for hidden carbs. Generally, dairy products and nuts are a good way to meet your daily fat intake, but know that some of those items may contain more carbohydrates than you think. For example, yogurt topped with nuts may seem like a great keto-friendly snack, but a 5.3 ounce serving of plain yogurt has 12 grams of carbohydrates. Vanilla flavored yogurt has 24 grams of carbohydrates. Add an ounce of cashews, weighing in at nearly nine grams of carbs, and you’re up to 21 to 33 grams of carbs for that snack, which could knock you out of ketosis. Be sure to read nutrition labels carefully and pay careful attention to serving sizes. Scientists published the first pilot study on the relationship between the ketogenic diet and cancer in 2011. For their study, they recruited 16 patients (12 women and 4 men) with various cancers in advanced stages. They had an average age of 50.4 years (30-65 years). All of them had metastatic tumors, which means that they grew and spread throughout the body.
Dr. Kossoff is a Professor of Neurology and Pediatrics at Johns Hopkins University in Baltimore, MD.  He received his medical degree from SUNY at Buffalo School of Medicine in New York, followed by a residency in pediatrics at Eastern Virginia Medical School in Norfolk, Virginia. He completed a fellowship in child neurology and then pediatric epilepsy and clinical neurophysiology at The Johns Hopkins Hospital in Baltimore.  He has been at Johns Hopkins since 1998.
More research is needed to determine whether the kind of extreme carb restriction associated with keto diets is necessary to unlock all these benefits, especially if you’re healthy. “Ketogenic and other very-low-carbohydrate diets can be quite challenging to follow over the long term, and the possibility of adverse effects has not been ruled out,” says Dr. David Ludwig, a professor of nutrition at Harvard School of Public Health. “Usually, such severe restriction isn’t necessary.” He points out that not all carbs are equal, and that the speed with which a carbohydrate food affects your blood sugar—what’s known as its glycemic index—makes a difference.
Further, the authors revealed that only 60 to 65% of patients with epilepsy become seizure free using medication while 35% are resistant to the effects of medication. And they used these statistics to justify this study. They further stated that there has been an “exponential” growth in interest in using the ketogenic diet for the treatment of epilepsy.

The oxygen-free pathway used by cancer cells needs a lot more glucose to produce the same amount of energy as an oxygen-using cell. This realization led to imaging techniques where cancer cells can be detected by spotting where large amounts of a glucose tracer accumulate in the body. The glucose used in this technique contains a small amount of radioactive material that lights up in the image.


A small Feb. 20, 2017, study looked at the impact of a six-week ketogenic diet on physical fitness and body composition in 42 healthy adults. The study, published in the journal Nutrition & Metabolism, found a mildly negative impact on physical performance in terms of endurance capacity, peak power and faster exhaustion. Overall, researchers concluded, “Our findings lead us to assume that a [ketogenic diet] does not impact physical fitness in a clinically relevant manner that would impair activities of daily living and aerobic training.” The “significant” weight loss of about 4.4 pounds, on average, did not affect muscle mass or function.
So far the research has found energy restriction to significantly reduce growth and progression of numerous cancers including mammary, brain, colon, pancreas, lung, and prostate cancer. However, it is important to note that the best results are achieved from severe calorie restriction (<1,000 calories per day).
