Hi Stacey, I can’t give medical advice and definitely recommend following your doctor’s recommendations. You can ask him/her if low carb would be better suited for you. Also, you may want to double check with him/her if the kidney concern was related to high protein, because that is a common misconception about keto – it is not a high protein diet/lifestyle.
Jimmy Moore: I wouldn’t fret so much about the blood sugar, I would definitely keep an eye on insulin. Unfortunately there’s just not an easy way to prick your finger and check insulin at home. It doesn’t exist yet. It’s coming real soon. It’s not available yet. I think the big take home is relax, chill out, you’re doing good for your body, especially like we talked about with the cholesterol numbers. If that HDL is going up, if the triglycerides are going down, the small LDL particles are going down, your HSCRP is under 1, why are you worried?
Great article! I have been diagnosed with familial hypercholesterolemia, however, I am not concerned at all. I continue to follow a restricted calorie, zero carb ketogenic diet, that I have been doing strictly for the last year and a half. All of my health markers are excellent, except for LDL particle number and total, and it is the large buoyant type. I am middle-aged (46). I did get both a CT scan and tri-vascular scan, and got perfect scores for both. That alleviated any concerns that I might have previously had. Since this diet has improved my gut issues, bodyfat, blood pressure, insulin, blood glucose, mood, energy, A1C, CRP, etc., - basically every single health parameter I have tested, I am sticking with it! Thank you for clearing up the confusion surrounding LDL!
I just got to know too that my Cholesterol raised a lot, I've been on keto for a little more than 3 months and before that always had a low carb diet. I don't know if the 3 day fast I did before taking the blood test only " aggravated" the situation. The total is 302, LDL is 214! But since the triglycerides are 94 and HDL 57, it seems to be OK. I read that a low rate of TG/HDL may indicate that the LDL particles are of the bigger, fluffier type less dangerous.

In laboratory animals, a liver stem cell placed into the bone marrow starts creating not liver, but bone marrow cells, a bone marrow stem cell transplanted into the liver begins to generate not bone marrow, but liver cells. Look, the good doctor is right – he only forgot to stress “portion control” which is why many fanatical dieters are so kee-jerk reactive to any discussion – odds are you over ate like a hog before your keto diet, and are weak and insecure in your diet plans. Eat EVERYTHING in small amounts, and you will live long and prosper. The only thing to avoid are processed foods. Cook your meals from scratch using quality ingredients.
Your current cholesterol levels l is higher than I would personally feel comfortable with. I would consider making a few dietary changes (i.e., increasing fiber and net carbs, reducing saturated fat, and increasing protein), especially given your lack of improved cognition and decreased ability to work out. When you’re eating the foods that get you there (more on that in a minute), your body can enter a state of ketosis in one to three days, she adds. During the diet, the majority of calories you consume come from fat, with a little protein and very little carbohydrates. Ketosis also happens if you eat a very low-calorie diet — think doctor-supervised, only when medically recommended diets of 600 to 800 total calories.
The biggest shifts in your daily habits will be how you food shop and how you cook, and recipes that are ketogenic need to be followed rather than just low-carb. You will require the healthy fats in order to get into ketosis and have enough energy without the carbs. And you will be considerably more energetic and healthier when cooking your own keto-friendly food rather than buying supposedly keto foods off the shelf.
The ketogenic diet achieved national media exposure in the US in October 1994, when NBC's Dateline television programme reported the case of Charlie Abrahams, son of Hollywood producer Jim Abrahams. The two-year-old suffered from epilepsy that had remained uncontrolled by mainstream and alternative therapies. Abrahams discovered a reference to the ketogenic diet in an epilepsy guide for parents and brought Charlie to John M. Freeman at Johns Hopkins Hospital, which had continued to offer the therapy. Under the diet, Charlie's epilepsy was rapidly controlled and his developmental progress resumed. This inspired Abrahams to create the Charlie Foundation to promote the diet and fund research.[10] A multicentre prospective study began in 1994, the results were presented to the American Epilepsy Society in 1996 and were published[17] in 1998. There followed an explosion of scientific interest in the diet. In 1997, Abrahams produced a TV movie, ...First Do No Harm, starring Meryl Streep, in which a young boy's intractable epilepsy is successfully treated by the ketogenic diet.[1]
The ketogenic diet (also known as the "keto diet") is a high fat, low carbohydrate, and "just enough" protein for growth and maintenance diet. Typically, it follows a ratio of 3-4 grams of fat for every 1 gram of carbohydrate and protein. This means that about 90% of the calories come from fat, requiring those on the diet to eat mostly fatty foods such as butter and cream. Although it may sound terrible, there are many creative recipes that make this a tasty approach. What distinguishes the traditional ketogenic diet from the modified-Atkins diet is the rigorous attention to food intake and limitation on total calories.
It’s also interesting that the cancers used to produce the basic science cited by Dr. Seyfried are virtually all brain cancers and that virtually all the evidence comes from rodent tumor models. For one thing, if there is a tumor type that exhibits the Warburg effect and a high degree of metabolic derangement, it’s brain tumors. It’s no coincidence that dichloroacetate was first tested in brain tumors. In this study, VM/Dk mice were used, and a mouse histiocytoma cell line resembling human glioblastoma multiforme with macrophage/microglial properties derived from that same mouse strain (VM-M3) was implanted subcutaneously. This cell line has the property of metastasizing quickly and widely when implanted under the skin and allowed to grow, which actually makes it not very much like brain tumors, which seldom metastasize and usually kill through local invasion and taking up increasing volume in the closed space of the skull, something the brain most definitely does not like. The results showed that a ketogenic diet increased mean survival time by over 56%, while a combination of a ketogenic diet and hyperbaric oxygen therapy (HBOT) increased survival time 78%. The result is interesting, but it is a mouse tumor model, not a human tumor model, and that makes its applicability to humans tenuous, particularly given the nature of the murine tumor, but probably worth investigating further.
Jimmy Moore: One thing we’ve not talked about, I alluded to a little bit earlier, is LDL particle size. When people hear LDL, they think “Oh that’s one number.” Uh-uh, it’s actually 2 major numbers, it’s a lot more than 2. It’s about 6 or 8 sub-fractions of LDL sizes. Primarily it’s pattern A and pattern B. Pattern A being the large fluffy kind. Pattern B being the small dense kind.
• Increasing muscle mass — Jeff Volek, Ph.D., is a registered dietitian specializing in how a high-fat, low-carb diet can affect health and athletic performance. He's written many scientific articles on this topic, as well as two books, and he explains that ketones have a similar structure to branched-chain amino acids that can be useful for building muscle mass. Ketones spare these amino acids, leaving higher levels of them around, which can help promote muscle mass.
As of the moment, there is no industry standard as to how many calories should be consumed in a restricted ketogenic diet, but there are published studies that provide estimates. In one example, a 65-year-old woman who was suffering from glioblastoma multiforme (GBM), an aggressive type of brain cancer, was put into a restricted ketogenic diet that started with water fasting and then proceeded to consuming 600 calories a day only.
The weight and body mass index of the patients decreased significantly (P<0.0001). The level of total cholesterol decreased from week 1 to week 24. HDL cholesterol levels significantly increased, whereas LDL cholesterol levels significantly decreased after treatment. The level of triglycerides decreased significantly following 24 weeks of treatment. The level of blood glucose significantly decreased. The changes in the level of urea and creatinine were not statistically significant.
The next case report is from 2010. It describes the case of a 65-year-old woman who presented with progressive memory loss, chronic headaches, nausea, and a right hemisphere multi-centric tumor seen with magnetic resonance imaging (MRI). Following incomplete surgical resection, the patient was diagnosed with glioblastoma multiforme (GBM). Now here’s the kicker: The patient underwent standard therapy plus the ketogenic diet. A day after her surgery, she underwent a two-day fast, followed by a three day fast beginning a week after surgery, followed by a restricted ketogenic diet (only 600 Cal/day). Three weeks after her surgery (and two weeks after starting the ketogenic diet) she began standard of care treatment, concomitant radiation plus chemotherapy (temozolomide), “according to standard procedures,” which lasted six weeks. The patient also had a gene mutation in her tumor that produces increased sensitivity to temozolomide. The conclusion? Fortunately for the patient, she had what appears to have been a complete response, after which she went on a less restrictive ketogenic diet. Unfortunately, the patient recurred eight months later. By that point, the patient was off of the ketogenic diet. The authors’ conclusion? Because it was “unlikely” that the tumor would have responded this well on standard therapy alone, it must have been adding the ketogenic diet that done it. Worse, in the talk, Dr. Seyfried strongly implies that the tumor recurred because she had gone off the ketogenic diet two and a half months before her recurrence.

I’m 50 and have been working out with weights and cardio for over 30 years, 6-7 days per week. I have always had controlled weight in the 180’s for 5′ 9″ due to strength training. I went on a “version” of the Keto diet—per day goals: 35 net carbs/85-95 grams protein/35-40 grams fat. I didn’t want to go all-in with high fats. I weighed 188 at the start and in 6 months dropped 26 lbs to 162, and bodyfat at 9%. My cholesterol levels have mostly been 220 with 50-75 HDL.

I wanted to put it out there that I made this meal plan specifically with women in mind. I took an average of about 150 women and what their macros were. The end result was 1600 calories – broken down into 136g of fat, 74g of protein, and 20g net carbs a day. This is all built around a sedentary lifestyle, like most of us live. If you need to increase or decrease calories, you will need to do that on your own terms.
Meat products make up a big part of the keto diet, but experts stress the importance of choosing quality. This essential, fully referenced book is a practical guide for physicians, patients and caregivers, and provides step-by-step instructions for customizing the diet and clear explanations of the cutting-edge research on ketogenic therapies being done by Dr. Dominic D’Agostino’s team at the University of South Florida and Dr. Thomas Seyfried’s team at Boston College. The ketogenic diet for cancer is based on the consumption of whole, fresh foods and it can be used in addition to standard care or as a stand-alone treatment in wait-and-see situations.
The MAD aims to provide increased flexibility and palatability, with a 1:1 ratio of fat to carbohydrates and protein, and contains around 65% fat, 25% protein, and 10% carbohydrate (Payne et al., 2018). Fat is encouraged and the carbohydrate intake is limited to 10–20 g/day in children and 15–20 g/day in adults (Kossoff, 2004; Kossoff and Dorward, 2008). Because of carbohydrate restriction, the MAD can also produce urinary ketones (Carrette et al., 2008). The MAD does not require weighing food on a gram scale, or restriction of calories, protein or liquids, and may be a good option for patients who are unable to tolerate a more restrictive diet such as the classical ketogenic diet (KD) (Cervenka et al., 2012). Low-carbohydrate multivitamin and calcium carbonate supplementation is recommended in the MAD (Kossoff et al., 2009).
What are those lines of evidence? Well, there are drugs which lower insulin and glucose, like metformin. It’s a commonly used drug in diabetes that has shown promising results in cancer treatment. This is not fringe stuff here. Metformin is being studied. There is an article about it for cancer treatment on the NIH Cancer Institute website and on the MD Anderson website, which are two prominent mainstream cancer treatment organizations. There are several studies to support that as well.
Leanne: Yeah totally, chill out. Do a little meditation if you need to get into the zone. Exactly. You mentioned vegetable oils causing inflammation, are there specific fats that will help us, like we’ve talked a little bit about saturated fats, we all know trans fats don’t even go there … the mono poly saturated, unsaturated, is there a certain ratio that we should be aiming for. I know you’re the guy that will just slice off butter and eat it … I am too, a little bit of Himalayan rock salt on there never hurt anyone. Are there specific facts that we should be focusing on?
According to the USDA, a 100-gram serving of red tomatoes has 3.89 grams of carbohydrates.43 You may add this fruit to your ketogenic diet safely and gain its beneficial nutrients, particularly lycopene. Researchers from Ohio State University suggest that this antioxidant may help protect your skin from sun damage, which may result in a lowered risk of skin cancer tumors.44

It’s a habit to enjoy a brie cheese for desert instead of a piece of chocolate cake but each are favored deserts in France. I’m personally more satisfied after a 350 calorie sized wedge of brie than the same number of calories of cake.. which will give me sugar crash and .. really I’d like two slices of cake(I’ve got a sweet tooth that once I get going it wants to keep being fed)

Jimmy Moore: That’s a loaded question and we could probably go 3 hours just on that but I’ll give you the nut shell. Most medical professionals have been taught to look at 2 primary numbers on your cholesterol panel. Total cholesterol, and this number called LDLC, which is your LDL cholesterol that you’ve heard about. You’ve heard LDL is bad and you’ve heard if you’re total cholesterol level is over 200, oh my god you are at great risk for heart disease. Exactly. 

Another epilepsy syndrome in which the diet may be particularly useful is Dravet syndrome (also known as severe myoclonic epilepsy of infancy). This syndrome is classically described as a prolonged febrile seizure in the first 2 years of life, followed by focal-onset seizures, myoclonus, and developmental delays [16, Class III]. Dravet syndrome is associated in many cases with mutations in the gene SCN1A, a subunit of the sodium channel [17].


The oxygen-free pathway used by cancer cells needs a lot more glucose to produce the same amount of energy as an oxygen-using cell. This realization led to imaging techniques where cancer cells can be detected by spotting where large amounts of a glucose tracer accumulate in the body. The glucose used in this technique contains a small amount of radioactive material that lights up in the image.
A: The most common ways to track your carbs is through MyFitnessPal and their mobile app. You cannot track net carbs on the app, although you can track your total carb intake and your total fiber intake. To get your net carbs, just subtract your total fiber intake from your total carb intake. I have written an article on How to Track Carbs on MyFitnessPal.

Since any unplanned eating can potentially break the nutritional balance required, some people find the discipline needed to maintain the diet challenging and unpleasant. Some people terminate the diet or switch to a less demanding diet, like the modified Atkins diet or the low-glycaemic index treatment diet, because they find the difficulties too great.[42]
In one hypoxia-ischemia model, rats fed a ketogenic diet for 25 days before cardiac arrest had fewer postarrest seizures and myoclonic jerks and less neurodegeneration (determined by Fluoro-Jade staining) than those fed a normal diet [28,29]. The ketogenic diet also appears to have cardioprotective properties in an isolated heart perfusion model designed to mimic global ischemia. These changes were concomitant with increased numbers of mitochondria in cardiac muscle, suggesting that improved capacity to generate energy conferred a protective effect in the face of an ischemic insult [30].
Why does everyone simply condemn meat as a possible cause of cancer? It’s not about the meat of chickens or beef but the way they are raised these days! Farms raised chickens are in such tight quarters that they are not even able to sit down and are prone to disease. So what do they do? Feed them “antibiotics”! Then they feed them pesticide laden food! Then to quickly fatten them up they are given hormones to promote growth. Beef/cows are also living in crowded barns and fed grain with growth hormones added and pesticide laden HMO grain. Is it any wonder that we are exposed to getting cancer? If we all are outraged by this practice and we should be, we would be switching to organic meat, raised cage free without pesticides, hormones, or antibiotics. Some companies and food markets are beginning to realize this as a problem and have greatly enlarged their organic sections lately! Check your labels when shopping!
Animal research indicates that a ketogenic diet reduces levels of brain amyloid-beta, a misfolded protein that contributes to Alzheimer’s disease, while also restoring mitochondrial function and improving learning and memory. (18, 19, 20) Although fewer studies on a ketogenic diet have been done in humans with Alzheimer’s disease, a recent trial found a ketogenic diet to be both safe and effective for mild Alzheimer’s disease. (21)

This has been very comforting for me to read as I’ve been on a Keto diet for 8mo, loving my results which were weight loss (initially) but now the sustained clarity & sharpness of mind. I am lean (58kg) and very active. I had a blood test last week just to see where I’m at & my LDL is very high. Of course my doctor wants me to stop Keto but I just feel so great. I’m torn...
firstly, the LMHR profile also applies to long-term fasting studies - this is perhaps where it can be observed in its natural, unconfounded state - LDL rises in lean healthy individuals during a fast, but drops or stays stable in those with obesity or atherosclerosis (period varies from 3-21 days). This is consistent with the keto pattern; this evidence clearly shows that LDL divergence is related to burning a high % of fat, and not to eating it, which is to some extent a confounder with keto (as shown by the cholesterol drop protocol and the possible different effects of different fat types and amounts).
Here’s what happens when you eat low carb, high fat, keto. The small LDLP number goes way down. Along with the triglycerides dropping, along with the HDL going up, along with all those other great markers that improve that nobody’s paying attention to in the medical profession, your small LDL goes down. The question that comes into play here is “What about the number of total particles?” That’s the debate that nobody’s going to answer until we do some studies on it.

Leanne: I went to my naturopath, and she’s like this is awesome, this is exactly what we want to see. My issue was hormones, so I want cholesterol so that my body can build hormones, because without that cholesterol good luck. When I went to my regular GP, he was like “Statins, put you on statins. Oh my gosh it’s way too high.” What do you say to people, you’re saying 200 is always thrown out there with cholesterol. What you’re saying is that cholesterol is not really all that important in the grand scheme of things if you’re looking at your number. It’s more important to look at triglycerides in that ratio?

[57:08] – While the experiment worked, in that it showed a decrease in lipid levels, other markers such as postprandial glucose levels increased. Dave talks about the importance of informing yourself, finding a balance and listening to how the body feels. He also talks about plans to continue experimenting with different levels but taking into account different risk-factors. Ultimately, he prefers a lower ketogenic ratio. You may also be concerned about the effects of eating all that butter and cream. After all, isn't fat supposed to be bad for you? A recent study confirmed that children on the ketogenic diet do have significantly higher levels of cholesterol than most kids. But damage from a high-fat diet generally comes only after many years. Children usually follow the ketogenic diet for just a few years.
Unfortunately that’s wrong. Unfortunately total cholesterol doesn’t tell the whole story. It includes one number that you want to have higher. That is you HDL good cholesterol. When you start eating low carb, high fat, keto, one of the tell tale signs that you’ve done it very well is your HDL, especially you ladies, you’re lucky Leanne … you ladies can make your HDL just go really high. I have to work hard as a guy to get mine in the 70-80 range, which is pretty good for a guy. Most people walking around their HDL is sub 40, and most of them probably sub 20 if they’re not eating enough fat. You have to eat saturated fat in order to raise that good HDL cholesterol. Okay?

However, you might be reluctant to make any changes to your diet given the benefits you've experienced. On the other hand, you may decide that you want to try to lower your LDL values while still following a keto/low-carb lifestyle.
Leigh Tracy, RD, a dietitian and certified diabetes educator at The Center for Endocrinology at Mercy Medical Center in Baltimore, also advised people to remember that the study did not focus on whether or not the keto diet prevented or inhibited cancer growth — only on changes to the body including the fact that the diet reduced that patients’ central body fat, improved insulin levels, and improved lean body mass.
