Because epilepsy is a metabolic disease (Clanton et al., 2017), interest in studies of alterations of metabolism by anticonvulsants such as the KD has increased, as has their importance for the treatment of drug-resistant epilepsy. This contribution reviews the use and effects of the KD and its variants for the treatment of adults and children with intractable epilepsy.
Russel Wilder first used the ketogenic diet to treat epilepsy in 1921. He also coined the term "ketogenic diet." For almost a decade, the ketogenic diet enjoyed a place in the medical world as a therapeutic diet for pediatric epilepsy and was widely used until its popularity ceased with the introduction of antiepileptic agents. The resurgence of the ketogenic diet as a rapid weight loss formula is a relatively new concept the has shown to be quite effective, at least in the short run.
Additionally, they are composed of higher lipid and lower protein content than LDL. Because of their physical properties and functional purposes, VLDL particles are more likely than other lipoproteins to clog vessels and impair vascular functions. Research studies have noted that high levels of VLDL are associated with increased risk of artherosclerosis and cardiovascular diseases. [20] VLDL is also considered to be a more precise indicator than LDL-C for a variety of metabolic conditions. [21]
In fact, a lot of people suffer from low levels of HDL cholesterol, which prevents their body from clearing out cholesterol from the blood. Most Americans don’t have enough HDL to decrease their risk of cardiovascular disease[ix]. Crazy enough, low cholesterol levels are actually associated with increased mortality from stroke and heart disease[x].
I’m not going to shell out 20k to know my LDL-P. I’d rather assume that I’m part of the 30% who reacts to saturated fat with elevated LDL-P. Should I be worried, considering all my other markers point to the right direction? Expert after expert say that we don’t know what these mixed signals mean until there is a study of people doing low-carb high-fat diets, correlating their lipid profiles with incidence of cardiovascular disease.
Long-term use of the ketogenic diet in children increases the risk of slowed or stunted growth, bone fractures, and kidney stones.[18] The diet reduces levels of insulin-like growth factor 1, which is important for childhood growth. Like many anticonvulsant drugs, the ketogenic diet has an adverse effect on bone health. Many factors may be involved such as acidosis and suppressed growth hormone.[38] About one in 20 children on the ketogenic diet develop kidney stones (compared with one in several thousand for the general population). A class of anticonvulsants known as carbonic anhydrase inhibitors (topiramate, zonisamide) are known to increase the risk of kidney stones, but the combination of these anticonvulsants and the ketogenic diet does not appear to elevate the risk above that of the diet alone.[39] The stones are treatable and do not justify discontinuation of the diet.[39] Johns Hopkins Hospital now gives oral potassium citrate supplements to all ketogenic diet patients, resulting in one-seventh of the incidence of kidney stones.[40] However, this empiric usage has not been tested in a prospective controlled trial.[9] Kidney stone formation (nephrolithiasis) is associated with the diet for four reasons:[39]
So rather than giving one-size-fits-all dietary advice or weaponizing the word “balanced” it might be better if the medical community suggested that there are Individual differences that need to be considered. This might also help those lay folk who have had success with one dietary lifestyle or another also realize that what’s valid for them may not be good advice for others.
There were some commonalities among the diets, of course; all these traditional people ate some animal products, and all consumed a fair amount of fat, whether from plant or animal sources. All the food was, of course, locally grown, locally harvested, or locally hunted, since these isolated groups lacked access to the industrialized food of modern “civilization.”

But in Dr. Price’s day, many groups living in many different locations still lived according to tradition largely untouched by modern Western influence. Price’s travels took him from the Eskimos of the Arctic, to the descendents of the Incas living in the high Andes, to the Masai on the plains of Kenya, to isolated Swiss herders in the Alpine mountain valleys, to Polynesians living on pristine tropical islands.
Data on the efficacy of KD and the modified Atkin’s diet (MAD) in children and adults are similar.1 “Approximately one-third of adults with epilepsy that does not respond to medications may have a 50% seizure frequency reduction. Five to 10% of these adults may have a dramatic response, with greater than or equal to 90% seizure frequency reduction. This is similar to the response seen in children,” said Klein.

As I ponder this enthusiasm, I have to think that perhaps I am just a little slower, or more cautious, than most. The day after I first met Dr. Kelley in New York in July 1981, I was on a plane to Dallas to begin my review of Kelley’s charts. As previously discussed, I quickly found among Kelley’s records case after case of appropriately diagnosed poor-prognosis and/or terminal cancer, patients alive five, ten, even 15 years later, with no possible explanation for such survival other than Kelley’s odd nutritional treatment.


Now that we have a superficial understanding of the total-to-HDL cholesterol ratio, let’s look at the results of the meta-analysis. The two findings that the researcher’s highlight most are the effects that lauric acid (found in high quantities in coconut oil) and stearic acid (found in high quantities in animal fats) have on the total-to-HDL cholesterol ratio. Both of these fatty acids improved the total-to-HDL cholesterol ratio more favorably than carbohydrates [25]. (A similar pattern emerges for blood sugar and triglyceride levels when we replace carbs with fat as well.)


I had been working with Dr. Donna Andrews and was able to understand that my seizures had to do with a lot more than the former doctors had explained to me. I was learning about how my blood sugar and stress levels affected my health and had experienced multiple stretches of three months without seizures but it was never consistent. Even though I knew I was headed in a better direction, I didn’t believe I could really be well.
In 2005, he would eventually die with his dream of academic acceptance unrealized. But my colleague Dr. Linda Isaacs and I have worked diligently over the past 26 years, keeping the Kelley idea alive, that different people may require completely different diets. In the next installment, I will address my own experience treating patients diagnosed with advanced cancer with a Kelley based approach. Our therapy involves, oftentimes, diets high in carbohydrates, which proponents of the ketogenic diet would predict should fuel, not stop, cancer.
Ketone bodies, especially β-hydroxybutyrate, can be measured easily, so much work has centered on determining how these molecules may have anticonvulsant effects. Inconsistencies in studies attempting to correlate seizure protection with levels of ketone bodies suggest that another mechanism may be involved in the diet’s beneficial effects on seizures [2–5, Class III]. Several mechanisms have been proposed, including changes in ATP production making neurons more resilient in the face of metabolic demands during seizures; altered brain pH affecting neuronal excitability; direct inhibitory effects of ketone bodies or fatty acids on ion channels; and shifts in amino acid metabolism to favor the synthesis of the inhibitory neurotransmitter GABA [6,7].

Keto will increase your cholesterol, because your body is getting more cholesterol from food. So guess what your body will do when you increase the dietary cholesterol…that’s right, it’ll stop producing its own cholesterol. Since 75% of your cholesterol is made inside your body, if you increase the amount that you eat, your body will cut that 75% down. Your body isn’t stupid. It knows what it needs.
I am trying to get back into keto. I did it before and I was so happy when I lost 10lbs (I did the keto for a month). I am ready to go back to this lifestyle. Ketogenic diets (some using calorie restriction) have been associated with decreased tumor growth in animal models of gliomas [10], prostate cancer [25], and gastric cancer [11]. In the context of cancer, ketone bodies may provide an alternative substrate for ATP production in malignant cells, as outlined above. However, other work suggests that glucose is used to produce components critical to proliferative cell growth [26], and it is conceivable that the ketogenic diet may restrict that aspect of malignant cell transformation.
The CKD and its variants should be considered as an alternative for non-surgical pharmacoresistant patients with epilepsy, of any age. Each patient must have an individually designed diet; however, adult patients have more difficulty in maintaining the CKD. It is essential to inform the patient and the family about the efficacy and AE related to the KD, and the use of websites and videos may help in this education. Although several theories exist regarding the mechanisms of action, further study is needed nevertheless the positive results are probably due to several mechanisms.

Senior moments. They joke about, “Oh you’re just getting older so you’re going to get dementia, you’re going to get Alzheimer’s, that’s just normal of getting older.” No! No it’s not. They’re taking these very drugs that are robbing the body of exactly what it needs to stay mentally sharp. I’m in my early forties now, I’m hoping twenty, thirty years from now … having all that butter I eat, and healthy fats is going to keep this noggin sharp. I think it will and I’ll be danged if I ever take anything that will take away the very key element that’s making my brain sharp.
Serious complications of the ketogenic diet are rare and have only been described in single reports, including those of Fanconi’s renal tubular acidosis (in co-treatment with valproate), severe hypoproteinemia, marked increase in liver function tests (in co-treatment with valproate), cardiomyopathy, prolonged QTc, acute hemorrhagic pancreatitis, basal ganglia injury, scurvy, lipoid pneumonia, and propofol infusion syndrome.
Nevertheless, anti-angiogenesis as the answer to cancer remains a big driving force in “biotech” companies, who have developed a whole slew of angiostatin and endostatin offspring, including the drug Avastin, costing up to $10,000 a month, though it doesn’t work particularly well. The clinical studies aren’t impressive, usually reporting several months of improved survival in patients diagnosed with a variety of advanced cancers.
I want to be very clear, though, that I don’t believe claims that are made on some websites that the ketogenic diet beats chemotherapy for all cancer treatment. There’s simply no research to support that. I don’t know where those websites are getting that idea, and there’s a lot of snake oil when it comes to cancer treatment out there. It’s a really vulnerable population. Someone who’s diagnosed with cancer, particularly a late-stage cancer that might be terminal, understandably we often feel pretty desperate and might not have the capacity at that moment in time to go through the proper vetting process to make sure that some of the more alternative therapies that are suggested are legitimate, and so you see a lot of wacky stuff recommended for cancer treatment.
The most common side effect encountered is constipation. Many of the children who begin the diet are already prone to this problem because of limited mobility, hypotonia, or spasticity. Constipation can be treated with regular doses of polyethylene glycol, fiber, increased fluids, salt, mineral oil, intermittent pediatric-dose enemas, or magnesium hydroxide . Other more common side effects include hunger, acidosis (during illness), and hypoglycemia (just during the start of the diet). Many children as well can have gastroesophageal reflux, which can be managed with medications. The high fat content decreases gastric emptying, which promotes gastroesophageal reflux.

Keto flu symptoms and side effects can include feeling tired, having difficulty sleeping, digestive issues like constipation, weakness during workouts, being moody, losing libido and having bad breath. Fortunately, these side effects don’t affect everyone and often only last for 1–2 weeks. (And yes, you CAN build muscle on keto.) Overall, symptoms go away as your body adjusts to being in ketosis.
A popular keto supplement are exogenous ketones (popularly called “keto diet pills”) that may help you achieve results earlier as well as remain in that state. (Don’t confuse exogenous ketones with raspberry ketones, as the latter don’t raise ketone levels in the body or mimic endogenous ketones, so you wouldn’t use raspberry ketones in your regimen.)
Ketogenic diets (KDs), being high in fat and low in carbohydrates, have been suggested to reduce seizure frequency in people with epilepsy. At present, such diets are mainly recommended for children who continue to have seizures despite treatment with antiepileptic drugs (AEDs) (drug‐resistant epilepsy). Recently, there has been interest in less restrictive KDs, including the modified Atkins diet (MAD), and the use of these diets has extended into adult practice. This is an update of a review first published in 2003 and last updated in 2016.

The classic ketogenic diet used as far back as the 1920s is a four to one ratio (in grams) of fat to carbohydrates plus protein. That’s about 90 percent of calories coming from fat. We typically teach our patients a modified Atkins diet, which limits the net carbohydrate intake to 20 grams per day without any limitations on the amount of protein and fat they eat. Typically, about 60 to 70 percent of their daily calorie intake comes from fat. (That’s more than double what the USDA Dietary Guidelines recommend.)

A study evaluating the effects of a ketogenic diet among 66 obese adults, from which 35 had high cholesterol levels and 31 had normal levels of cholesterol demonstrated that both groups resulted in statistically significant decreased levels of LDL cholesterol, total cholesterol, and triglycerides, whereas the HDL cholesterol levels were increased. These results show that keto diet can improve cholesterol levels and ratios of cholesterol levels among obese people regardless of their cholesterol levels before the dietary intervention. Furthermore, this study also demonstrates that low-carb diet is safe to use for a longer period of time in overweight people with a high total cholesterol level and those with normocholesterolemia (18).

In terms of weight loss, you may be interested in trying the ketogenic diet because you’ve heard that it can make a big impact right away. And that’s true. “Ketogenic diets will cause you to lose weight within the first week,” says Mattinson. She explains that your body will first use up all of its glycogen stores (the storage form of carbohydrate). With depleted glycogen, you’ll drop water weight. While it can be motivating to see the number on the scale go down (often dramatically), do keep in mind that most of this is water loss initially.
No single food can cure cancer, but some research has shown a link between the keto diet and slowed growth of some types of tumors in mice. A few studies in humans with certain types of brain tumors have also shown promise. On the contrary, a very low-fat diet has been found to reduce risk of recurrence for certain types of breast cancer. Some researchers are conducting more clinical trials with cancer patients, looking at how diet affects patients, along with chemotherapy and radiation. We hope that this research, as well as future research, will help us better understand the role that the keto plays in cancer.
Another possible concern with long term adherence to a ketogenic diet is that some people are ‘hyper-responders’ to high fat intake. In these people, (estimated to be around 25-30% of the population) a high fat intake may lead to increased cholesterol and triglyceride levels. While this may be cause for concern in some people, in others this increase might be temporary, and still others may find, after considering things such as inflammation levels, total cholesterol:HDL ratio, and triglyceride:HDL ratio, the increase is not cause for concern.
The ketogenic diet is not a benign, holistic, or natural treatment for epilepsy; as with any serious medical therapy, complications may result.[28] These are generally less severe and less frequent than with anticonvulsant medication or surgery.[28] Common but easily treatable short-term side effects include constipation, low-grade acidosis, and hypoglycaemia if an initial fast is undertaken. Raised levels of lipids in the blood affect up to 60% of children[38] and cholesterol levels may increase by around 30%.[28] This can be treated by changes to the fat content of the diet, such as from saturated fats towards polyunsaturated fats, and if persistent, by lowering the ketogenic ratio.[38] Supplements are necessary to counter the dietary deficiency of many micronutrients.[18]

So far the research has found energy restriction to significantly reduce growth and progression of numerous cancers including mammary, brain, colon, pancreas, lung, and prostate cancer. Your current cholesterol levels l is higher than I would personally feel comfortable with. I would consider making a few dietary changes (i.e., increasing fiber and net carbs, reducing saturated fat, and increasing protein), especially given your lack of improved cognition and decreased ability to work out. Also, consider supplementing with the amino acid leucine, as it can be broken down directly into acetyl-CoA, making it one of the most important ketogenic amino acids in the body. While most other amino acids are converted into glucose, the acetyl-CoA formed from leucine can be used to make ketone bodies. It’s also present in keto friendly foods like eggs and cottage cheese.
The KDE is usually begun in a hospital setting, and often begins with a one- to two-day fasting period (though there may be a trend away from both of these requirements). After determining the proper amount of protein (depending on age, etc.), the diet is structured as a ratio of fat grams to protein grams plus carb grams. It usually begins with a 4 to 1 ratio, and then can be fine-tuned from there. The diet is often calorie-limited and fluid-limited as well. Additionally, no packaged "low-carb foods" (shakes, bars, etc.) are allowed for at least the first month.
Over the past century, ketogenic diets have also been used as natural remedies to treat and even help reverse neurological disorders and cognitive impairments, including epilepsy, Alzheimer’s symptoms, manic depression and anxiety. Research shows that cutting off glucose levels with a very low-carb diet makes your body produce ketones for fuel. This change can help to reverse neurological disorders and cognitive impairment, including inducing seizure control. The brain is able to use this alternative source of energy instead of the cellular energy pathways that aren’t functioning normally in patients with brain disorders.
In this same chapter, there are also two case reports, neither very impressive. The first, written by the mother, tells the story of a four-year old child diagnosed in 2004 with a low-grade (less aggressive) but quite large and inoperable brain tumor. The parents, as the mother writes, entrusted their child into the hands of the experts, who prescribed the usual “gold standard” treatments, which are not clearly described initially but presumably mean chemotherapy and perhaps radiation.

Increased enterohepatic circulation on high fat means that cholesterol is kept "in play" - bounced back into the bloodstream in ApoB particles - while low enterohepatic circulation, in people with with higher synthesis rates, during weight loss - when cholesterol is being dumped by shrinking cells - means that cholesterol can pile up in the gall bladder faster than it can be conjugated to bile salts and bile acids and faster than it can be extracted by the weak stimulus of low fat food.
When mitochondria were isolated from these SOD1 mice, β-hydroxybutyrate rescued ATP production in the presence of a complex I inhibitor. It also helped to preserve neurons in culture exposed to the same inhibitor, paralleling the findings found for PD. In these neurodegenerative disorders, the ketogenic diet may be providing substrate to bypass impaired or poorly functioning complex I. Another hypothesis on enhanced ATP production includes increased mitochondrial biogenesis [48•]. Alternatively, decreased reactive oxygen species generation (which protects the process of oxidative phosphorylation) could be the result of an effect on NADH oxidation or preventing adverse events in the handling of calcium overload in mitochondria, such as the mitochondrial permeability transition [48•,49,50].
I have pancreatitis, well controlled, which is the way I want to keep it. The biggest difficulty I have with keto is this: I eat a small portion of steel cut oats in the morning. When I don’t, within two days , I start having bleeding, dark in colour. My endrocrinolagest feels that I need the roughage in the steel cut oats to replete the bowel lining. I have great difficulty loosing weight, always have, even though I eat very clean, no junk food, never eat out, don’t like pop, don’t crave sugar, cook all food fresh. Any comment? Willing to try anything you can suggest.

Ketogenic diets represent a far more effective strategy for managing type 2 diabetes than the American Diabetes Association’s high-carb, low-fat dietary guidelines. Unlike the ADA’s guidelines, a ketogenic diet significantly reduces blood sugar, hemoglobin A1c levels, waist circumference, and triglycerides in diabetic individuals. (13) Most importantly, research indicates that the diet is sustainable for diabetic patients and that the beneficial changes can be maintained over the long term. (14)
The mainstay of treatment for epilepsy is pharmaceutical intervention. As I recently noted, more and more we are seeing surgical procedures being performed for those individuals who have not had a significant improvement with drugs. I indicated that at least some individuals are gluten sensitive and may benefit from a gluten-free diet which potentially could keep them from undergoing potentially life-threatening surgery as a treatment for their epilepsy.
So what, exactly, does “ketogenic” mean? The name refers to a specific type of energy-carrying molecule, called a ketone. “Most people are always in a state of glucosis, meaning they’re burning glucose from carbohydrates for energy,” Westman says. “But you determine what your body burns for fuel based on what you feed it.” By severely restricting carbs and increasing your fat intake, your body can shift into a state of “ketosis,” which means it’s burning fat instead of glucose. “Ketosis used to be considered abnormal, but it can actually be very healthy,” Westman says.
Available research on the ketogenic diet for weight loss is still limited. Most of the studies so far have had a small number of participants, were short-term (12 weeks or less), and did not include control groups. A ketogenic diet has been shown to provide short-term benefits in some people including weight loss and improvements in total cholesterol, blood sugar, and blood pressure. However, these effects after one year when compared with the effects of conventional weight loss diets are not significantly different. [10]

They need to make a lot of ATP, and quickly, to support their high requirements for energy. Adenosine triphosphate, also known as ATP, is a compound that provides energy to drive hundreds of thousands of biochemical processes in living cells. Found in all forms of life, ATP is often referred to as the chemical energy “currency” that powers metabolic activity.
Children with epilepsy due to mutations in GLUT-1, which transports glucose across the blood-brain barrier, suffer from seizures in infancy. If not identified and treated, they develop microcephaly, mental retardation, spasticity, and ataxia as a consequence of relative brain hypoglycemia. These children respond well to the ketogenic diet, as it is believed to provide an alternative fuel source for their central nervous system [13, Class III].
Cyclical ketosis means you’re sometimes in ketosis and sometimes aren’t. A few days each week—the night before workout days to build glycogen stores in your muscles—try increasing your intake of berries, higher complex carb veggies (like sweet potatoes), and non-gluten grains. It might knock you out of ketosis temporarily, but it also provides a wealth of nutrients to keep you lean, healthy, and happy. This is also called flexible ketosis, which creates metabolic flexibility—the holy grail of metabolism management. I’ve also talked about cycling ketosis with intermittent fasting, which provides a win-win strategy to reach your health goals.
This device measures the amount of acetone (another ketone) you release in your breath when you are in ketosis. (Acetone is also the ketone responsible for bad breath when you’re in ketosis.) There are a number of kits on the market. One study showed they are as effective as ketone strips for testing ketone levels, but critics argue both have drawbacks including testing for only one specific ketone.
Feldman believes that his findings thus far demonstrate that the combination of higher energy demands, lower body fat stores, and lower glycogen stores in LMHRs trigger increased production of LDLs for the purpose of carrying energy (triglycerides) to cells that need them, with cholesterol mainly along for the ride but also used by the cells for repair and other purposes, as needed.
Instead of thinking about the total carbs you’re eating, assess what those carbs provide to you. Do the majority of your carbs come from fruit and vegetables, with a payload of fiber and disease-fighting antioxidants? Fantastic. Or are you consuming them in the form of added sugars (cookies, candy, soda) or refined flour? If you are, you know what to do.
So sugar feeds the cancer cells- you dont have to go full keto just stop eating sugar? I keep reading meat causes cancer and a vegan diet is best! When you cut out sugar and all grains you end up eating dairy and meat, with nuts for a treat. Friut is off the list, honey and some root veg like potatos so if you cut down on meat what the feck are we left with? you can only eat so many eggs,avocados and spinach. I also am yet to find a keto dessert I like, coconut flour and stevia dosent make a cake.

Medications may be tapered and discontinued on an individual basis because one of the most common parental reasons for starting the diet is anticonvulsant reduction. However, not all children are able to come off medications. Some child neurologists tell families that the diet and medications are often a “partnership” in seizure control . Children should be seen in clinic every 3 months for the first year, with more frequent visits for infants and medically fragile patients.
Some experts now feel that impaired energy metabolism may be the defining common factor in nearly all cancers, regardless of their origin.12,14 They believe that cancer, rather than being caused by errors in DNA, is fundamentally a metabolic disease caused by a disruption in a cancer cell’s ability to get the energy it needs from mitochondrial respiration like a normal cell would.

Aside from the various keto-friendly foods mentioned in this article, you may be wondering if there are other options that may help support your ketogenic diet. If you find that the ketogenic diet is limiting when you start out, don't worry. There's actually a lot you can add to your diet that's "keto" as long as consumption is controlled. In 2006, Strahlman [53, Class III] reported the case of his own wife, whose intractable migraine headaches resolved after a medically supervised low-calorie diet. Husain and colleagues [54, Class III] studied an Atkins diet–like plan in patients with narcolepsy and reported an 18% decrease in daytime sleepiness as measured by a standard questionnaire. The Atkins diet is less restrictive than the ketogenic diet and does not contain as much fat as “classic” ketogenic diets.
Also, consider supplementing with the amino acid leucine, as it can be broken down directly into acetyl-CoA, making it one of the most important ketogenic amino acids in the body. While most other amino acids are converted into glucose, the acetyl-CoA formed from leucine can be used to make ketone bodies. It’s also present in keto friendly foods like eggs and cottage cheese.
