A lot of people take their macros as a “set in stone” type of thing. You shouldn’t worry about hitting the mark every single day to the dot. If you’re a few calories over some days, a few calories under on others – it’s fine. Everything will even itself out in the end. It’s all about a long term plan that can work for you, and not the other way around.
I won’t sugar coat it. The diet can be hard to start. The first two weeks can be terrible. You give up a lot of comfort foods. Plus, you will need new cookbooks. So, switching to a ketogenic diet isn’t the first thing that pops into your head when you hear cancer. But the diet works. I steadily lost weight without substantial hunger or changes to my limited exercise program. My overall health improved, I slept better, felt better and hopefully look better.
Before starting the diet, the patient should maintain a seizure diary to establish a frequency parameter. Also needed are a laboratory evaluation including selenium and carnitine levels (Table 5), electroencephalogram (EEG), and a magnetic resonance image (MRI) of the brain. A renal ultrasound should be done in case of kidney stones; an electrocardiogram and carotid ultrasound are considered optional (Kossoff et al., 2018). The nutritional evaluation includes a nutritional anamnesis including a 3-day food report, food habits, allergies, aversions, and intolerances. Baseline weight, height, and the ideal weight for stature and body mass index (BMI) are needed to calculate the ketogenic ratio, calories, and fluid intake. The diet formulation should be established according to the patient’s age and the administration route (Kossoff et al., 2009).

A very strict form of the ketogenic diet has been used in epileptic patients longer term (several years) and is considered safe for that purpose. I’m following the ketogenic diet and I find it very easy, pleasant and varied. I can even say that my diet today is more varied than the previous one. I do not intend to leave this diet and I cannot really see why. My initial focus was not to lose weight, I’ve always been lean, but to feel better, well disposed. And I got it! I am very pleased, I have read a lot about it (including scientific literature) and I have influenced other people who need to lose weight or improve some aspects of their health. Drink lots of water. This is especially crucial on a low carb or keto diet. Why? When you eat carbohydrates, your body stores the extra as glycogen in the liver, where they are bound to water molecules. Eating low carb depletes this glycogen, which allows you to burn fat – but it also means you are storing less water, making it easier to get dehydrated. Instead of the traditional recommendation of 8 cups of water per day, aim for 16 cups when following a low carb lifestyle.

Anecdotally, I’ve spoken with some cancer researchers who claim to be virtually curing cancer in animals using a combination of ketogenic diet and PI3K or mTOR, like rapamycin, but these data aren’t published. Again, we need to be cautious about accepting these claims until they’ve gone through the legitimate scientific channels and people have had a chance to review this research.
The MAD aims to provide increased flexibility and palatability, with a 1:1 ratio of fat to carbohydrates and protein, and contains around 65% fat, 25% protein, and 10% carbohydrate (Payne et al., 2018). Fat is encouraged and the carbohydrate intake is limited to 10–20 g/day in children and 15–20 g/day in adults (Kossoff, 2004; Kossoff and Dorward, 2008). The MAD does not require weighing food on a gram scale, or restriction of calories, protein or liquids, and may be a good option for patients who are unable to tolerate a more restrictive diet such as the classical ketogenic diet (KD) (Cervenka et al., 2012). Low-carbohydrate multivitamin and calcium carbonate supplementation is recommended in the MAD (Kossoff et al., 2009).

Hi Gigi, Low carb and keto is about the balance of macronutrients eaten (fat, protein and carbs), not specifically meat or lack thereof. Most people on keto do eat meat, though some people do vegetarian keto. Fat is actually necessary for many body processes. There is no issue for the kidneys with a high fat diet, but if you eat too much protein that isn’t great for the kidneys. It’s a common misconception that keto is high protein (it isn’t). Keto is great for diabetics as it naturally helps stabilize insulin. These supplements—called exogenous ketones—aren’t necessarily bad, but neither are they a free pass to indulge and then effortlessly shift into ketosis.
In fact, ketogenic diets have been used for nearly a century to treat seizures, says Gary Yellen, a professor or neurobiology at Harvard Medical School. “It dates back to studies from the 1920s that found this kind of diet was like a sustainable form of fasting, which we’ve known, supposedly since antiquity, to be beneficial for epilepsy,” he says.
Therefore, when you’re following a ketogenic diet plan for beginners, your body is burning fat for energy rather than carbohydrates, so in the process most people lose weight and excess body fat rapidly, even when consuming lots of fat and adequate calories through their daily food intake. There are so many tricks, shortcuts, and gimmicks out there on achieving optimal ketosis – I’d suggest you don’t bother with any of that. These authors also observed an inhibitory effect of KB on mitochondrial permeability transition related to apoptotic and necrotic death. Moreover, in experimental models, acetoacetate exerted a broad-spectrum anticonvulsant effect (Rho et al., 2002). In another study, Rho (2017) described a relationship among KB, neurotransmitter release and ATP-sensitive potassium channels (Rho, 2017). Similarly, to these studies, injection of KB led to the reduction of seizure susceptibility (Gasior et al., 2008). Ma et al. (2007) found a decrease of the spontaneous firing rate in sections of mouse tissue, which was eliminated in the absence of ATP-sensitive potassium channels (KATP). In addition, KB can exert a direct inhibitory effect on the vesicular glutamate transport (Juge et al., 2010). It is possible that these divergent results are related to the different concentrations of KB used in these studies and the diverse seizure thresholds of the animal models. These conflicting results can be also explained by differences in diet composition.

Usually when the classic ketogenic diet is prescribed, the total calories are matched to the number of calories the person needs. For example, if a child is eating a 1500 calorie regular diet, it would be changed to a 1500 calorie ketogenic diet. This is the main reason why the USDA must continue supporting a low-fat diet and condemning saturated fats, even though the science does not support their positions. It is no surprise, therefore, to learn that peer-reviewed scientific studies continue to show that the high-fat ketogenic diet supports cognitive health and can help prevent or reduce cognitive diseases such as Alzheimer's. I’m 50 and have been working out with weights and cardio for over 30 years, 6-7 days per week. I have always had controlled weight in the 180’s for 5′ 9″ due to strength training. I went on a “version” of the Keto diet—per day goals: 35 net carbs/85-95 grams protein/35-40 grams fat. I didn’t want to go all-in with high fats. I weighed 188 at the start and in 6 months dropped 26 lbs to 162, and bodyfat at 9%. My cholesterol levels have mostly been 220 with 50-75 HDL.
What a great post. I thought i would add about the selection of food you eat on keto and that everyone is different. Some food gives you energy and some doesnt, this varies person to person. I started and quit keto 3 times before i managed to find my balance. The first few times it made be poorly, from the shock of diet change. However, you can wean yourself into the diet which i did the last time when i had the most success.

Leanne: Billion. Okay, let’s talk a little bit about Staten, I can’t remember what book it was but they were saying that cholesterol is really important for brain health. You mentioned that too, the cholesterol uses the nutrient and moves it up to your brain, it’s really important. What I was reading is that as we age our cholesterol actually increases slightly to help with that aging process. You often meet people that are in their 40s, 50s, I know a lot of people in my family are now on Staten because their cholesterol is increasing. I always say, “Maybe that’s your body’s way of protecting yourself against aging.”
They also noted that some patients were more responsive to the ketogenic diet than other patients were. The best response was in a 3-year-old girl who had complete remission five years of treatment with a ketogenic diet. Two other patients also experienced complete remission after the diet, and the other two patients had disease progression after stopping keto. (Keep in mind, however, that all of these patients used conventional treatments along with the keto diet.)
Although 86% of people who have undergone the protocol have experienced a decrease in LDL cholesterol after ramping up their calorie and fat intake for three days, my own LDL cholesterol increased, the first time by 47 mg/dL and the second time by 15 mg/dL. At this point, I'm the only one who had both an increase in LDL and a decrease in triglycerides after the high-fat, high-calorie portion of the experiment. Hi, I’m still a bit skeptical, I have seen some of my friends do the keto diet, and have had good results. Though I am still not sure about the idea of the fats being eaten. They say they eat meat with the fat and must do so, is this correct? Also isn’t this not good for the body especially for the kidneys? Second, can a diabetic do this diet? There are many questions running through my head.
In their conclusions, they stated that the favorable response could be attributed “in part” to the calorie-restricted ketogenic diet. However, the researchers emphasized that “further studies are needed to evaluate the efficacy of restricted ketogenic diets, administered alone or together with standard treatment, as a therapy for GBM and possibly other malignant brain tumors.”
In 1920 a New York physician, Dr. Galen, reported at the American Medical Association convention that he had had significant success in treating epilepsy by initiating a program of fasting. At that time the only pharmaceutical interventions that were available included phenobarbital and bromides. Interestingly, the patient he treated was actually a young cousin who had aggressive seizures. On the second day of fasting the child’s epilepsy abated and did not return over the next two years of follow-up. Further studies appearing in 1923, 1926, 1928, all confirmed the effectiveness of fasting as an effective treatment for seizures.

Another study from 2014 suggests that the ketogenic diet is a safe, effective treatment to manage aggressive cancers when used with conventional therapies. For their retrospective study, researchers reviewed 53 patients with glioma, a tumor that starts in glial cells in the brain and spinal cord. These patients were treated with chemoradiotherapy and other standard treatments from August 2010 to August 2013. Six of these patients also consumed a ketogenic diet while on treatment.
If you’ve decided to move forward in trying the keto diet, you will want to stick to the parameters of the eating plan. Roughly 60 to 80 percent of your calories will come from fats. That means you’ll eat meats, fats, and oils, and a very limited amount of nonstarchy vegetables, she says. One of my favorite patients, whom I have discussed at times in my lectures, was diagnosed in August 1991 with stage IV pancreatic cancer, with multiple metastases into the liver, into the lung, into both adrenals, and into the bone. The constant consumption of a high-carbohydrate diet causes the body to rely on glucose (sugar) for fuel, while rarely tapping into fat stores for energy. A ketogenic diet does just the opposite. It forces the body to turn to fats for fuel. A keto diet encourages the production of ketones, small water-soluble compounds, and the “burning” of fatty acids in adipose tissue (fat cells) for energy. Ketones are unique in that they are rapidly taken up by tissues and broken down to yield ATP, the primary energy currency of the human body. The process by which the body switches to using ketones for energy is referred to as “nutritional ketosis,” while the process of tapping into your body’s fat stores is termed “fat adaptation.”
There were adverse effects within all of the studies and for all KD variations, such as short‐term gastrointestinal‐related disturbances and increased cholesterol. However, study periods were short, therefore the long‐term risks associated with these adverse effects is unknown. Attrition rates remained a problem with all KDs and across all studies; reasons for this being lack of observed efficacy and dietary tolerance.

I just got to know too that my Cholesterol raised a lot, I've been on keto for a little more than 3 months and before that always had a low carb diet. I don't know if the 3 day fast I did before taking the blood test only " aggravated" the situation. The total is 302, LDL is 214! But since the triglycerides are 94 and HDL 57, it seems to be OK. I read that a low rate of TG/HDL may indicate that the LDL particles are of the bigger, fluffier type less dangerous.
Seyfried agrees that there is clear evidence that cancer is a genetic disease, since we can inherit mutations that are clearly associated with increased cancer risk. That’s not at all controversial. That’s well established, and even Seyfried agrees with that. But he argues that many of these mutations that we can inherit are mutations that actually disturb cellular respiration, maybe that the heritable aspect of cancer is not mutation that drives itself—cellular proliferation—but instead are mutations that actually cause mitochondrial dysfunction and defects in cellular respiration. He also points out that many of the non-inherited causes of cancer that have been identified and are clearly recognized, like radiation, impair mitochondrial function. That may be a common mechanism that is shared between these non-inherited causes of cancer and inherited causes of cancer.
High-protein ketogenic diet (HPKD): This version of the keto diet is often followed by folks who want to preserve their muscle mass like bodybuilders and older people. Rather than protein making up 20 percent of the diet, here it’s 30 percent. Meanwhile, fat goes down to 65 percent of the diet and carbs stay at 5 percent. (Caution: folks with kidney issues shouldn’t up their protein too much.)
Certainly Dr. Seyfried has put together a most impressive achievement, chronicling in great detail his belief that cancer does not develop from genetic alterations – as is generally believed – but as a result of changes in fundamental cell physiology, specifically changes in energy production, that in turn lead to the cancer phenotype. In essence, the genes remain intact, but metabolism goes awry.
