If you have a history of hypothyroid issues, you may also struggle with unhealthy cholesterol levels as well — and the keto diet can make them even worse. However, for those of you who are being treated for your hypothyroid condition or who have an autoimmune thyroid condition, you may be able to follow the keto diet without any problems. In fact, many keto dieters with autoimmune thyroid conditions have found that the keto way of eating improved their quality of life more than any other diet.
Wondering how many carb foods you can eat and still be “in ketosis”? The traditional ketogenic diet, created for those with epilepsy consisted of getting about 75 percent of calories from sources of fat (such as oils or fattier cuts of meat), 5 percent from carbohydrates and 20 percent from protein. For most people a less strict version (what I call a “modified keto diet”) can still help promote weight loss in a safe, and often very fast, way.

One approach to this question has been to examine the variations in the size of the LDL particles in the blood. With a well-formulated ketogenic diet, we see a shift away from the small dangerous LDL even when the total LDL goes up, so most of this increase is in the ‘good’ or ‘buoyant’ LDL fraction (Hallberg, 2018). Another factor to be taken into account is that during rapid weight loss, cholesterol that you had stored in your adipose tissue (ie, body fat) is mobilized as the fat cells shrink (Phinney 1990). This will artificially raise serum LDL as long as the weight loss continues, but it then comes back down once weight loss stops. To avoid being misled by this, the best strategy is to hold off checking blood lipids until a couple of months after weight loss ceases.
Regular follow-ups with the dietitian, and medical team, will monitor your or your child’s growth (height and weight, if applicable), health, their epilepsy, and if there is a need for any change to their anti-epileptic drugs (AEDs), such as changing to sugar-free versions. If the diet is followed carefully, individuals do not put on weight, or lose weight inappropriately.
Cyclical ketosis means you’re sometimes in ketosis and sometimes aren’t. A few days each week—the night before workout days to build glycogen stores in your muscles—try increasing your intake of berries, higher complex carb veggies (like sweet potatoes), and non-gluten grains. It might knock you out of ketosis temporarily, but it also provides a wealth of nutrients to keep you lean, healthy, and happy. This is also called flexible ketosis, which creates metabolic flexibility—the holy grail of metabolism management. I’ve also talked about cycling ketosis with intermittent fasting, which provides a win-win strategy to reach your health goals.

We also maintain a certain amount of stored sugar as glycogen, found in the liver and muscle and formed when glucose molecules link up to one another in complex chains. In times of need and if deprived of dietary carbohydrates, our liver and muscle cells can break down glycogen into glucose for release into the bloodstream. Our liver cells can also, when necessary, convert certain amino acids such as alanine into glucose.
Because it lacks carbohydrates, a ketogenic diet is rich in proteins and fats. It typically includes plenty of meats, eggs, processed meats, sausages, cheeses, fish, nuts, butter, oils, seeds, and fibrous vegetables. Because it is so restrictive, it is really hard to follow over the long run. Carbohydrates normally account for at least 50% of the typical American diet. One of the main criticisms of this diet is that many people tend to eat too much protein and poor-quality fats from processed foods, with very few fruits and vegetables. Patients with kidney disease need to be cautious because this diet could worsen their condition. Additionally, some patients may feel a little tired in the beginning, while some may have bad breath, nausea, vomiting, constipation, and sleep problems.
Epilepsy can be caused by a variety of different conditions including head trauma, infection, brain tumor, and stroke, but by and large most cases of epilepsy have no readily identifiable cause. Epilepsy affects some 2.3 million adults in America and close to half a million children. Further, about one in 26 people will be diagnosed with epilepsy at some point in their lives. It’s been estimated that there are approximately 150,000 new cases of epilepsy diagnosed in the United States each year and overall about $15.5 billion in medical costs as well as lost earnings and production are attributed to this disease.
For example, a pretty large number of animal studies have shown that a ketogenic diet can reduce tumor growth and improve survival rates. There was one 22-day study in mice that looked at the differences between the ketogenic diet and other diets. That study found that a ketogenic diet reduced tumor growth by up to 65% and nearly doubled survival time in some cases.
Over the past century, ketogenic diets have also been used as natural remedies to treat and even help reverse neurological disorders and cognitive impairments, including epilepsy, Alzheimer’s symptoms, manic depression and anxiety. Research shows that cutting off glucose levels with a very low-carb diet makes your body produce ketones for fuel. This change can help to reverse neurological disorders and cognitive impairment, including inducing seizure control. The brain is able to use this alternative source of energy instead of the cellular energy pathways that aren’t functioning normally in patients with brain disorders.
You may also be concerned about the effects of eating all that butter and cream. After all, isn't fat supposed to be bad for you? A recent study confirmed that children on the ketogenic diet do have significantly higher levels of cholesterol than most kids. But damage from a high-fat diet generally comes only after many years. Children usually follow the ketogenic diet for just a few years.

There are some treatment centers like Care Oncology Clinic in the UK and ChemoThermia Oncology Center in Istanbul that are using ketogenic diet and fasting along with glucose inhibitors and conventional treatment like chemo. They claim to be getting good results, but I don’t know much about these cancer centers above and beyond what I just told you. Note that keto only seems to work with the faster-growing cancers like breast cancer, but not as much with slower-growing cancers like prostate cancer.
• Fighting inflammation — The human body can use both sugar and fat as fuel sources. However, the latter is preferred because it is a cleaner, healthier fuel that releases far fewer reactive oxygen species (ROS) and secondary free radicals. By eliminating sugar from your daily food consumption, you're decreasing your risk of developing chronic inflammation throughout your body.

Eating a 5,000-calorie diet high in fat will certainly put you in ketosis, but you probably won’t lose weight because those excessive calories have to go somewhere. You needn’t be as adamant about calories doing a ketogenic diet, but they do still matter. Drizzling 500 calories of MCT oil or butter onto your steak will probably get you into ketosis, but the scales aren’t going to provide you much love. However, having the right amount of extra fat in the diet while keeping starchy carbohydrates low will make you into a fat-burning machine.
Although 86% of people who have undergone the protocol have experienced a decrease in LDL cholesterol after ramping up their calorie and fat intake for three days, my own LDL cholesterol increased, the first time by 47 mg/dL and the second time by 15 mg/dL. At this point, I'm the only one who had both an increase in LDL and a decrease in triglycerides after the high-fat, high-calorie portion of the experiment. All of my other markers, including HDL and small LDL particle counts, were excellent for both the low-calorie and high-calorie days.
• High-protein ketogenic diet — This method is a variant of the SKD. In a high-protein diet, you increase the ratio of protein consumption to 10 percent and reduce your healthy fat consumption by 10 percent. In a study involving obese men that tried this method, researchers noted that it helped reduce their hunger and lowered their food intake significantly, resulting in weight loss.11
Anecdotally, I’ve spoken with some cancer researchers who claim to be virtually curing cancer in animals using a combination of ketogenic diet and PI3K or mTOR, like rapamycin, but these data aren’t published. Again, we need to be cautious about accepting these claims until they’ve gone through the legitimate scientific channels and people have had a chance to review this research.
"The keto diet is primarily used to help reduce the frequency of epileptic seizures in children. While it also has been tried for weight loss, only short-term results have been studied, and the results have been mixed. We don't know if it works in the long term, nor whether it's safe," warns registered dietitian Kathy McManus, director of the Department of Nutrition at Harvard-affiliated Brigham and Women's Hospital.
The popular belief that high-fat diets cause obesity and several other diseases such as coronary heart disease, diabetes, and cancer has not been observed in recent epidemiological studies. Studies carried out in animals that were fed high-fat diets did not show a specific causal relationship between dietary fat and obesity. On the contrary, very-low-carbohydrate and high-fat diets such as the ketogenic diet have shown to beneficial to weight loss.

Something that makes the keto diet different from other low-carb diets is that it does not “protein-load.” Protein is not as big a part of the keto diet as fat is. Reason being: In small amounts, the body can change protein to glucose, which means if you eat too much of it, especially while in the beginning stages, it will slow down your body’s transition into ketosis.
Although Kelley did prescribe a variety of diets for his cancer patients, these two exemplary patients followed a plant-based eating plan, high in carbohydrates with a minimum each day of four glasses of carrot juice, dense in nutrients but also dense in natural sugar. Each of these diets allowed considerable fruit and whole grain products, foods again loaded with carbs. According to Seyfried’s hypothesis, both should have died quick miserable deaths.

So what do you do about GBM? Standard treatment begins with surgery. After surgery, you are given radiation and chemo. In the meantime, you take other medications to control the side effects. Tick, tick, tick, GBM makes you acutely aware of clocks ticking. You start searching for medical trials. There are many rules to qualify, most extend life by only a few months. Some have a substantial chance of killing you.
Taken together, results from preclinical studies, albeit sometimes contradictory, tend to support an anti-tumor effect rather than a pro-tumor effect of the KD for most solid cancers. However, even though pro-tumor effects are rare, they cannot be ruled out per se. Most importantly, available preclinical evidence implies that the feasibility of a KD as an adjuvant cancer therapy strongly depends on the type of tumor and its genetic alterations.
In a model of PD, neurons cultured from the developing mesencephalon, the site of the future substantia nigra, are susceptible to injury and death from the application of 1-methyl-4-phenylpyridinium (MPP+), which inhibits mitochondrial energy production. Adding one of the ketone bodies, β-hydroxybutyrate, rescues these cells from death and reduction in neurite outgrowth [37]. In an in vivo model, mice treated with β-hydroxybutyrate via continuous subcutaneous infusion were relatively protected from the dopaminergic degeneration induced by injection of MPTP, an MPP+ precursor, apparently by enhancing oxidative phosphorylation and the production of ATP [38].

Conversely, fenofibrate, not used clinically as an antiepileptic, exhibits experimental anticonvulsant properties in adult rats comparable to the ketogenic diet.[58] This, together with studies showing its efficacy in patients who have failed to achieve seizure control on half a dozen drugs, suggests a unique mechanism of action.[56]

In the study, Barbara A. Gower, Ph.D., and her colleagues analyzed data from 45 women diagnosed with ovarian and endometrial cancers. The women were randomly assigned to either a ketogenic or a standard, healthy diet group. The ketogenic group was asked to consume 70 percent of calories from fat, 25 percent from protein and 5 percent from carbohydrates. The comparison diet was one recommended by the American Cancer Society, high in whole grains and fruit and low in added sugar.
The second is called LDL-P which measures the number of LDL particles in the blood. Sometimes, there is a correlation – more LDL particles means that you can have higher levels of LDL-C. However, larger LDL molecules can grow and carry more cholesterol – leading to a discordance in which LDL-C and LDL-P are not necessarily proportional. When this happens, LDL-C and LDL-P are said to be “discordant.”
While KD and MAD have demonstrated efficacy in reducing the extent and degree of seizures in epileptic patients, the dietary approaches have been tied to risk of adverse events in both adults and children, although most are treatable and are nearly all  preventable. Adverse effects include constipation, low blood sugar, and gastrointestinal reflux.5,6 Other side effects, such as kidney stones and high cholesterol, can often be improved with supplements or dietary changes.1 These also tend to improve over the long term. Kossoff and colleagues found that both total cholesterol and LDL cholesterol normalized within a year of treatment after initial increases during the first three months of MAD.5

The ketogenic diet keeps this process going. It forces the child’s body to burn fat round the clock by keeping calories low and making fat products the primary food that the child is getting. In fact, the diet gets most (80 percent) of its calories from fat. The rest comes from carbohydrates and protein. Each meal has about four times as much fat as protein or carbohydrate. The amounts of food and liquid at each meal have to be carefully worked out and weighed for each person.
Similar results were reported by a telemedical guided weight loss program among 200 overweight adults, where triglyceride and HDL-cholesterol levels were lower at month 6 in the keto diet group compared to the low-fat group. The conclusion was that a low-carb diet has more benefits in reducing cardiovascular risks compared to the low-fat diet (12).

I recently applied for life insurance after following the ketogenic diet for about six months. I was initially quoted the lowest rate based on the fact that I have no health issues whatsoever. However, my cholesterol readings were very high so they came back and said that I had elevated total cholesterol readings of 378 which alarmed me. They have now doubled my life insurance rates because of it. Even though my total cholesterol was high everything else seems good according to this article. LDL – 272, HDL – 92, Triclycerides – 70. This all translates to an LDL/HDL ratio of slightly under 3:1 and a Triglyceride/HDL ratio of close to 1:1. I don’t know if I should be concerned that my total is well over the 300 that is sited in this article. Does anyone know?


Its hard to find any information about hyper responders, even harder for me as my total cholestorl levels increased extremely after I went on the keto diet, from an already high 5mmol/dL to extremely high (14mmol/dL or 538mg) which is unheard of, even in the many hyper responder cases I've studied. But my HDL increased to 2.7mmol and my trigicerides stayed the same at good 0.9. Nobody 've seen has such a high total cholestrol. Even as i research how cholestrol doesn't have much link to heart disease mortality, there's no research on anything as high as my case.
A ketogenic diet promotes healing of the brain following TBI by increasing the activity of genes genes involved in energy metabolism, stimulating the generation of new mitochondria, and inhibiting the production of damaging reactive oxygen species in the brain. (24) In animal models of TBI, a ketogenic diet reduces cerebral edema and neuronal cell death while improving behavioral outcomes. (25, 26) While individual success stories of people using a ketogenic diet for TBI are easy to find on the internet, formal clinical trials are still needed.
The ketogenic diet reduces seizure frequency by more than 50% in half of the patients who try it and by more than 90% in a third of patients.[18] Three-quarters of children who respond do so within two weeks, though experts recommend a trial of at least three months before assuming it has been ineffective.[9] Children with refractory epilepsy are more likely to benefit from the ketogenic diet than from trying another anticonvulsant drug.[1] Some evidence indicates that adolescents and adults may also benefit from the diet.[9]

Studies assessing the efficacy of the MAD reported seizure freedom rates of up to 25% and seizure reduction rates of up to 60% in children. One study used a simplified MAD (sMAD) and reported seizure freedom rates of 15% and seizure reduction rates of 56% in children. One study utilised a MAD in adults and reported seizure reduction rates of 35%, but no patients became seizure free (GRADE rating low).


The body needs bile to break down and digest dietary fat, and the gallbladder is responsible for storing bile before its release into the small intestine. Removal of the gallbladder and gallbladder disease cause fat malabsorption and may make it difficult to follow a ketogenic diet. If you have had your gallbladder removed or have existing gallbladder disease, consult with your doctor before trying a ketogenic diet.
Although many hypotheses have been put forward to explain how the ketogenic diet works, it remains a mystery. Disproven hypotheses include systemic acidosis (high levels of acid in the blood), electrolyte changes and hypoglycaemia (low blood glucose).[19] Although many biochemical changes are known to occur in the brain of a patient on the ketogenic diet, it is not known which of these has an anticonvulsant effect. The lack of understanding in this area is similar to the situation with many anticonvulsant drugs.[56]

Fanatic? Someone with T2D, a disease usually claimed to be progressive and a never ending stream of problems and medications, was REVERSED. That’s something to shout from the rooftops. The drop in medication use alone, but the big pharma companies would prefer that people’s stories of reversing (well, putting it into remission) T2D get called fanatical instead of insightful.


I actually went on a ketogenic diet last year to see if it would help my migraines. I have a history of chronic migraines which would usually last 3 days, sometimes longer. Triptans help a lot but I don’t like having to take them. I stayed in ketosis for about 8 months and experienced a significant reduction in migraines, from feeling some type of headache (mild o r severe) almost everyday to 1 or 2x per month while in ketosis. Although I’m very healthy otherwise, I do think my migraines may have something to do with blood sugar fluctuations (despite previously eating a whole foods diet and no refined carbs), and keto totally stabilized this. I eventually came off of Keto because I’m not really a meat lover. When I came off, but remained low carb, my migraines stayed under control for the most part. When I increase carbs, they do return.

Ketogenic diets (KDs), being high in fat and low in carbohydrates, have been suggested to reduce seizure frequency in people with epilepsy. At present, such diets are mainly recommended for children who continue to have seizures despite treatment with antiepileptic drugs (AEDs) (drug‐resistant epilepsy). Recently, there has been interest in less restrictive KDs, including the modified Atkins diet (MAD), and the use of these diets has extended into adult practice. Seyfried agrees that there is clear evidence that cancer is a genetic disease, since we can inherit mutations that are clearly associated with increased cancer risk. That’s not at all controversial. That’s well established, and even Seyfried agrees with that. But he argues that many of these mutations that we can inherit are mutations that actually disturb cellular respiration, maybe that the heritable aspect of cancer is not mutation that drives itself—cellular proliferation—but instead are mutations that actually cause mitochondrial dysfunction and defects in cellular respiration. He also points out that many of the non-inherited causes of cancer that have been identified and are clearly recognized, like radiation, impair mitochondrial function. That may be a common mechanism that is shared between these non-inherited causes of cancer and inherited causes of cancer.
Recent studies show that low-carb diets such as keto are more effective at raising good (HDL) cholesterol than low-fat diets [1, 2]. However, there are also studies showing that keto can increase total cholesterol (HDL and LDL) [3]. On the other hand, low-carb, high-fat diets also decrease LDL particle concentration (LDL-P), increase the size of LDL cholesterol and decrease the amount of harmful VLDL cholesterol in the blood [2], all of which have a positive effect on cardiovascular fitness.
Metabolic Syndrome and Diabetes – Several studies have evaluated the effects of a low carbohydrate ketogenic diet on type 2 diabetes, with promising results. Among the benefits seen are improved blood glucose profiles, improved insulin sensitivity, improvements in hemoglobin A1c, decreased triglycerides, increased HDL cholesterol (commonly thought of as ‘good’ cholesterol), and an improved total cholesterol to HDL ratio.23 24 25 26 The aforementioned improvements not only support those with existing type 2 diabetes, but also those with metabolic syndrome.

Regular readers of SBM should know the problem with this sort of approach. No IRB worth its salt would approve such a trial because it would be ethically dubious, but, even worse, it would be ethically dubious and it wouldn’t really tell us anything unless those few patients either had near-miraculous responses or died very quickly. Anything else would simply tell us that the diet is probably doing no harm. More numbers would be needed, particularly if the comparison is to historical controls, to get even an inkling of whether there might be benefit. In that case, you might as well do a proper phase I/II clinical trial, which is what is happening. For instance:
Avoid Oxidized Fats – this will cause more oxidation and inflammation again. Fats are prone to oxidation by sunlight, heat, and general processing. That’s why you want to avoid all processed vegetable oils, trans fats, margarine like wildfire. Making sure you don’t exceed the smoking point of oils is also a protective measure. Don’t cook with olive oil or eat rancid fish oil supplements.
HDL cholesterol transfers the extra cholesterol from the blood to the liver for removal from the body and is also referred to as “good” cholesterol. LDL cholesterol transfers the cholesterol within the bloodstream to the parts of the body where cell repair is needed and also inside the arteries. It is also known as “bad” cholesterol. Total cholesterol is HDL and LDL cholesterol combined (4).
The nerve impulse is characterised by a great influx of sodium ions through channels in the neuron's cell membrane followed by an efflux of potassium ions through other channels. The neuron is unable to fire again for a short time (known as the refractory period), which is mediated by another potassium channel. The flow through these ion channels is governed by a "gate" which is opened by either a voltage change or a chemical messenger known as a ligand (such as a neurotransmitter). These channels are another target for anticonvulsant drugs.[7]

The ketogenic diet is not a benign, holistic, or natural treatment for epilepsy; as with any serious medical therapy, complications may result.[28] These are generally less severe and less frequent than with anticonvulsant medication or surgery.[28] Common but easily treatable short-term side effects include constipation, low-grade acidosis, and hypoglycaemia if an initial fast is undertaken. Raised levels of lipids in the blood affect up to 60% of children[38] and cholesterol levels may increase by around 30%.[28] This can be treated by changes to the fat content of the diet, such as from saturated fats towards polyunsaturated fats, and if persistent, by lowering the ketogenic ratio.[38] Supplements are necessary to counter the dietary deficiency of many micronutrients.[18]


After my original lengthy conversation with Dr. Kelley, my research mentor Dr. Good suggested that during my summer break I begin an informal review of Kelley’s patient charts located in his Dallas office. From my first day in Dallas, I found among Kelley’s records patient after patient with appropriately diagnosed poor prognosis or what would be considered terminal disease such as metastatic pancreatic and metastatic breast cancer, who had done well under his care for many years, often with documented regression of his disease.

Jimmy Moore: That’s a loaded question. I’ll try to make it a little more pointed. Cholesterol and I’m really happy to hear that you went from what you say around 140 or so, to about 230 or 40. That’s actually a very good thing, and one of the key elements in the body, one of the key roles of cholesterol in the body, is for hormone regulation. Vegan ketogenic diet or vegetarian diet: Yes, both are possible. Instead of animal products, plenty of low-carb, nutrient-dense vegan and/or vegetarian foods are included. When there is ample glucose available (derived from dietary carbohydrates), the body will use that glucose as its main fuel source for producing ATP (the body’s main energy currency). Small amounts of glucose can be stored, but once incoming glucose has been used up and those stores run out, the body shifts gears and increases the use of fatty acids (derived from body fat stores or dietary fats) to produce energy. While most cells can directly use free fatty acids in order to produce sufficient energy, fats cannot cross the blood-brain barrier making them inaccessible to the brain as a fuel source. To overcome this, the liver converts fat into acetoacetate, acetone and beta-hydroxybutyrate (a.k.a. ketone bodies or ketones) which can be used by red blood cells and cells of the central nervous system (including the brain) as an alternative energy source when glucose levels are low. Importantly, the vast majority of other bodily tissues can also use ketones to produce energy. In fact, ketone production is a normal part of healthy metabolism and small amounts of ketones are produced regularly—such as during extended exercise or during an overnight fast. The day before admission to hospital, the proportion of carbohydrate in the diet may be decreased and the patient begins fasting after his or her evening meal.[19] On admission, only calorie- and caffeine-free fluids[37] are allowed until dinner, which consists of "eggnog"[Note 8] restricted to one-third of the typical calories for a meal. The following breakfast and lunch are similar, and on the second day, the "eggnog" dinner is increased to two-thirds of a typical meal's caloric content. In contrast to the safe application of KDs reported in various cancer models, our research group recently reported that mice bearing renal cell carcinoma xenografts and with signs of Stauffer’s syndrome experienced dramatic weight loss and liver dysfunction when treated with a KD [4]. Another study investigating the effect of long-term KD treatment on kidney cancer described a pro-tumor effect of the KD in a rat model of tuberous sclerosis complex [5]. Most concerning is the observation that, in a mouse model of BRAF V600E-positive melanoma, tumor growth was significantly increased under the KD [6]. Moreover, the study also demonstrated that the ketone body acetoacetate stimulated the oncogenic signaling of the BRAF pathway. In contrast, the KD had no effect on the progression of NRAS Q61K-positive or wild-type melanoma xenografts [6]. Notwithstanding these observations, in a feasibility trial involving a limited number of patients with advanced malignancies, a patient with BRAF V600E-positive/BRAF-inhibitor resistant melanoma seemed to benefit from the KD [7].

I also might offer a thought as to why, from a more esoteric, more biochemical perspective, for most people diagnosed with cancer the ketogenic diet might not work. For the past 150 years, researchers have approached cancer as a disease in which perfectly happy, normal mature cells sitting in some tissue somewhere suddenly go awry, lose their normal regulatory restraint, develop a primitive, undifferentiated appearance or phenotype, begin proliferating without restraint, begin invading through tissues and organs, begin migrating, spreading, creating new blood vessels along the way to feed the rapacious appetite of cancer. But over the past 15 years, gradually, a new, more productive, and I believe more truthful hypothesis has emerged, spearheaded particularly by Dr. Max Wicha at the University of Michigan.
